随机比较恩丹西酮与胃复安并小剂量地塞米松控制大剂量顺铂为主的化疗所致的迟缓呕吐

Randomized Trial to Compare the Effect of Ondansetron versus Metopromide plus Dexamethasone in Controlling Delayed Emesis after High-Dose Cisplatin

目的:随机观察恩丹西酮与胃复安并小剂量地塞米松控制大剂量顺铂为主的化疗所致的迟缓呕吐。方法:79例经组织学或细胞学检查确诊为肺癌或乳腺癌者,接受大剂量顺铂(90～100mg/m2))为主的联合化疗。随机分为两组,A组37例于顺铂后8小时开始口服恩丹西酮(8mg,q8h,dl～3),B组42例于顺铂后8小时开始口服胃复安(20mg,q8h,dl～3)并地塞米松(l.5mg,q8h,dl～3)。结果:A组控制迟缓呕吐完全缓解率(CR)为29.7%,基本控制率(CR+PR)为70.3%。B组各为52.4%和88.1%,两组比较均有显著差异(P<0.05)。结论:顺铂后d2～6口服胃复安及地塞米松控制迟缓呕吐疗效优于口服恩丹西酮。

Objective:This is a randomized trial to compare the efficacy of ondansetron versus metoclopramide plus dexamethasone in controlling delayed emesis after cisplatin treatment.Methods:Seventy-nine patients with histologically or cytologically diagnosed advanced lung cancer or breast cancer treated with high-dose cisplatin-based chemotherapy were eligible for study.All the patients were randomized into two groups.Group A(37cases)received ondansetron8mg o-rally Q8h d1～d38hours after the use of cisplatin.Group B(42cases)received metoclopramide20mg plus dexamethasone1.5mg orally Q8h for three days.Results:The CR of delayed eme-sis in group A was29.7%and CR+PR was70.3%,while in group B they were52.4%and88.1%respectively with significant difference(P<0.05).Conclusion:The results showed that de-layed emesis occurred less frequently in the patients treated with metoclopramide plus dexametha-sone than in those treated with ondansetron.
