优化小视野扩散加权成像在胰腺病变中的应用 被引量:3

摘要 目的:对比分析优化小视野扩散加权成像(FOCUS DWI)与常规DWI在胰腺病变中的应用价值。方法:纳入44例疑似胰腺病变的患者,分别进行常规DWI和FOCUS DWI成像,对两种成像方式的图像质量进行评分,并分别计算相应的ADC值。分析比较两种成像评分及ADC值。结果:常规DWI信噪比(SNR)、分辨率及病变显示、伪影及变形方面评分分别为2.9±0.9、3.2±1.2、3.0±0.9,而FOCUS DWI的相应得分为3.3±0.7、3.7±0.8和3.8±0.7。FOCUS DWI的病变显示、伪影及变形得分均优于常规DWI(P<0.01),而图像SNR差异无统计学意义。正常胰腺、胰腺炎症和胰腺肿瘤通过常规DWI和FOCUS DWI得到的ADC值差异亦无统计学意义。结论:FOCUS DWI可以减轻变形和伪影,提高成像分辨率,在胰腺病变的诊断及鉴别诊断方面有一定的临床价值。 Objective:The purpose of this study was to investigate and compare the application of FOCUS DWI with conventional diffusion weighted imaging in the diagnosis of pancreatic diseases.Methods:44 patients with suspected pancreatic lesions were included in this study and underwent both FOCUS DWI and conventional DWI.Image quality of the two imaging methods was scored and the corresponding apparent diffusion coefficient(ADC)values were calculated.Imaging method scores and ADC values were compared and tested statistically.Results:The scores of conventional DWI signal-tonoise ratio(SNR),resolution and lesion demonstration,artifact and distortion were 2.9±0.9,3.2±1.2 and 3.0±0.9,while the corresponding scores of FOCUS DWI were 3.3±0.7,3.7±0.8 and 3.8±0.7,respectively.The FOCUS DWI lesions,artifacts and distortion scores were superior to conventional DWI(P〈0.01),and the image SNR ratio was not statistically different.There were also no significant differences in ADC values obtained from conventional DWI and FOCUS DWI in normal pancreas,pancreatic inflammation,and pancreatic tumors.Conclusion:FOCUS DWI can both reduce the distortion and artifacts and improve the imaging resolution which has certain clinical value in the diagnosis and differential diagnosis of pancreatic lesions.
