摘要
目的:探讨肥胖对哮喘预测指数(API)阳性患儿预后转归的影响。方法:选取2015年1月-2017年9月本院收治的肥胖API阳性患儿50例作为肥胖组,选取同期非肥胖API阳性患儿50例作为非肥组。两组均给予常规综合治疗。比较两组肺功能、治疗效果、急诊就诊、再住院情况。结果:肥胖组和非肥组治疗后达峰时间比(Tpef/Te)、达峰容积比(Vpet/VTex)、呼出50%潮气容积的呼吸流速比(TEF50/TIF50)均明显高于治疗前,非肥组治疗后TPTEF/Te、VPTEF/Ve、TEF50/TIF50均明显高于肥胖组,差异有统计学意义(P<0.05)。非肥组治疗总有效率明显高于肥胖组,差异有统计学意义(X^2=4.762,P<0.05)。随访6个月期间,非肥组急诊就诊率、再住院率均明显低于肥胖组,差异均有统计学意义(P<0.05)。结论:API阳性患儿经综合治疗后可缓解其肺功能,但肥胖可能会抑制综合治疗对患儿肺功能的改善作用,降低了治疗效果,不利于患儿的预后转归。
Objective:To discuss the effect of obesity on prognosis of children with API positive.Method:A total of 50 children with obesity and API positive in our hospital from January 2015 to September 2017 were selected as the obese group.In the same period,50 children with non-obese and API positive were selected as the non-obese group.Both groups were given routine comprehensive treatment.The lung function,therapeutic efficacy,emergency treatment and readmission were compared between the two groups.Result:The Tpef/Te,Vpet/VTex and TEF50/TIF50 of the two groups after treatment were significantly higher than those before treatment, Tpef/Te,Vpet/VTex and TEF50/TIF50 of the non-obese group were significantly higher than those of the obese group,the differences were statistically significant(P〈0.05).The total effective rate of non-obese group was significantly higher than that of obese group,the difference was statistically significant(X^2=4.762,P〈0.05).During the 6 months follow-up,the rate of emergency visit and re-hospitalization in the non-obese group were significantly lower than those in the obese group,the differences were statistically significant(P〈0.05).Conclusion:The comprehensive treatment can alleviate lung function of API positive children.But obesity may inhibit the improvement of lung function of children by comprehensive treatment.It reduces the therapeutic effect and is harmful to the prognosis of children.
