超声引导下前列腺穿刺活检与前列腺癌根治术后病理Gleason评分的对比分析

Comparison of Gleason Score between Ultrasonic Guiding Biopsy Specimens and Prostatecyomy Specimens

目的对比分析超声引导下前列腺穿刺活检术与前列腺癌根治术后的病理组织Gleason评分之间的差异。方法比较分析个旧市人民医院2015年1月至2016年12月95例行超声引导下前列腺穿刺活检的患者的Gleason评分与其前列腺癌根治术后的Gleason评分之间的差异。结果 95例患者中,51例(53.7%)根治术后与活检术Gleason评分相符,29例(30.5%)根治术后评分升高,15例(15.8%)根治术后评分降低,卡方检验二者差异,P=0.024,差异有统计学意义。结论超声引导下10针法前列腺穿刺活检的Gleason评分仍有一定局限性,临床医师根据此方法评价病情,选择治疗方案时,要充分考虑。

Objective To contrast Gleason score of ultrasonic guiding biopsy specimens with the final pathology results after radical prostatectomy. Methods From January 2015 to December 2016, 95 cases patients underwent ultrasound-guided prostate biopsies with 10-core method and radical prostatectomy for prostate carcinoma and the Gleason score of pathology results was analyzed. Results The Gleason score was identical in 51 cases（53.7%）. An upgrading in the radical prostatectomy specimens occurred in 29 cases（30.5%）, a downgrading in 15 cases（15.8%）. The P value of Chi-square test for the differences of Gleason score is 0.024. Conclusion The reliability of Gleason score of needle biopsy in predicting final pathology was limited.It should be considered in evaluating severity of prostate carcinoma.
