比索洛尔治疗维持性血液透析患者高血压的疗效观察

Effect of Bisoprolol Treatment for Hypertension in Maintenance Hemodialysis Patients

目的探讨比索洛尔治疗维持性血液透析患者高血压的临床疗效。方法选择2014年10月至2016年10月在双流区第一人民医院长期血液透析的患者52例为研究对象,按照随机数字表法将患者分成观察组与对照组,每组26例。对照组给予常规治疗,包括利尿剂、血管紧张素Ⅱ受体拮抗剂或血管紧张素转换酶抑制剂、钙离子拮抗剂。观察组在对照组治疗的基础上给予比索洛尔口服,开始剂量为5 mg qd;如血压未降至正常,每隔1周加2.5 mg qd,最大剂量为10 mg qd。疗程均为12周。观察两组患者血压、心率及药物不良反应。结果观察组总有效率为92.3%(24/26),对照组总有效率为76.9%(20/26),两组疗效比较差异有统计学意义(P<0.05),观察组血压下降幅度显著优于对照组(P<0.01),观察组心率较治疗前降低(P<0.05),两组不良反应发生无统计学差异(P>0.05)。结论比索洛尔治疗维持性血液透析患者高血压安全、有效。

Objective To study the clinical efficacy of Bisoprolol in the treatment of hypertension in maintenance hemodialysis patients. Methods A total of 52 patients with long-term hemodialysis were selected from October 2014 to October 2016 in the Shuangliu District First People＇s Hospital. The patients were divided into observation group and control group according to the random number table method. The control group was given conventional therapy, including diuretics, angiotensin II receptor antagonists or angiotensin converting enzyme inhibitors, and calcium antagonists. The observation group was given bisoprolol on the basis treatment of the control group, the starting dose was 5mg qd, if blood pressure did not drop to normal, every 1 week plus 2.5mg qd, the maximum dose is 10mg qd. The course of treatment was 12 weeks. The blood pressure, heart rate and adverse drug reactions were observed. Results The total effective rate was 92.3% （24/26） in the observation group and 76.9% （20/26） in the control group. There was significant difference between the two groups （P〈0.05）, The decrease of blood pressure in the observation group was significantly better than that in the control group （P〈0.01）. The heart rate of the observation group was lower than that before treatment （P〈0.05）. Adverse reactions were no significant difference between the two groups （P〉0.05）. Conclusion Bisoprolol is safe and effective in the treatment of maintenance hemodialysis patients with hypertension.
