摘要
目的采用Meta分析系统评价18氟代脱氧葡萄糖正电子发射计算机断层显像(18 F-FDG PET/CT)对肾上腺肿瘤良恶性的诊断价值。方法计算机检索PubMed、EMbase、The Cochrane Library、中国知网(CNKI)、万方、中文科技期刊等数据库,查找有关18F-FDG PET/CT对肾上腺肿瘤良恶性诊断性试验研究文献。由2名研究员按同一标准分别进行文献筛选、提取资料和评价文献质量,采用Meta DiSc 1.4软件进行Meta分析。结果最终纳入20篇文献,肾上腺肿瘤患者1 481例,肾上腺病变共计1 663个。Meta分析结果示,PET/CT对肾上腺肿瘤良恶性的诊断灵敏度为0.92,特异度为0.92,阳性似然比为9.54,阴性似然比为0.09,合并效应量为139.60,受试者曲线下面积为0.9708。18F-FDG在肿瘤摄取值/肝脏摄取值>1时代谢参数SUVmax比值的诊断效能最高(灵敏度为0.95、特异度为0.93、受试者曲线下面积为0.9789)。结论 18F-FDG PET/CT代谢参数中SUVmax比值(tumor/liver>1)对肾上腺肿瘤的良恶性诊断效能最高。
Objective To evaluate the diagnostic value of 18 fluorodeoxyglucose positron emission computed tomography(18 F-FDG PET/CT)in the diagnosis of benign and malignant adrenal tumors.Methods PubMed,EMbase,the Cochrane Library,Chinese Knowledge Network(CNKI),Wanfang and Chinese scitech periodicals were searched to identify the diagnostic tests of the benign and malignant adrenal tumors by 18 F-FDG PET/CT.According to the same criteria,two researchers conducted literature screening,data extraction and quality evaluation respectively.Meta DiSc1.4 software was used for meta analysis.Results Totally 20 articles including 1 481 adrenal tumors and1 663 adrenal lesions were included.Meta analysis showed that the sensitivity,specificity,positive likelihood ratio and the negative likelihood ratio of PET/CT to benign and malignant adrenal tumors were 0.92,0.92,9.54 and 0.09,respectively;the combined effect was 139.60,and the area under the curve of the subjects was 0.9708.When the ratio of 18 F-FDG in tumor uptake value/liver intake was more than 1,the diagnostic efficiency of the metabolic parameter SUVmax ratio was the highest(the sensitivity was 0.95,the specificity 0.93,and the area under the curve of the subjects 0.9789).Conclusion The diagnostic efficiency of SUVmax ratio(tumor/liver 1)is the highest in the diagnosis of adrenal tumors among the metabolic parameters of 18 F-FDG PET/CT.
