膝关节镜下后交叉韧带重建术的疗效分析

目的分析膝关节镜下后交叉韧带重建术的临床疗效。方法收集2013年1月-2016年6月医院收治的38例PCL撕裂伤且行膝关节镜下后交叉韧带重建术患者的临床资料,术后均给予早期物理疗法及康复训练,均完成术后6～18月随访调查,统计手术并发症发生率,记录患者手术前后不同时间视觉模拟评分(VAS)、膝关节Lysholm评分及国际膝关节评分委员会(IKDC)评分结果,总结膝关节镜下后交叉韧带重建术的临床价值。结果所有患者手术切口均Ⅰ期愈合,后抽屉试验与Lachman试验均为阴性,末次随访患者膝关节活动度平均(138.6±3.4)度,无1例发生早期并发症;术前,患者VAS评分较高,Lysholm评分及IKDC评分较低,术后6月、12月、18月,患者VAS评分降低,Lysholm评分及IKDC评分上升,与术前比较差异有统计学意义(P<0.05)。结论膝关节镜下后交叉韧带重建术治疗PCL撕裂伤效果肯定,可改善患者膝关节功能,重建膝关节稳定性,且并发症发生率低,安全性高。

Objective To （PCL） reconstruction. Methods analyze the clinical curative effect of The clinical data of 38 patients with knee arthroscopic posterior cruciate ligament PCL lacerations and undergoing knee arthroscopic PCL reconstruction in the hospital from January 2013 to June 2016 were collected. All patients were given early physical therapy and rehabilitation training after surgery, and they were followed up for 6-18 months after surgery. The incidence of surgical complications was statistically analyzed. Scores of visual analogue scale （VAS）, kneejoint Lysholm scores and scores of International Knee Documentation Committee （IKDC） were recorded before and after operation. The clinical value of arthroscopic PCL reconstruction was summarized. Results All surgical incisions healed by first intention, and the posterior drawer test and Lachman test were negative. At the last follow-up, the average range of motion of the knee joint was （138.6 ± 3.4） degrees, and there was no early complications. Before surgery, the VAS score was relatively higher while Lysholm score and I KDC score were relatively lower. VAS scores decreased while Lysholm score and IKDC score increased at 6 months, 12 months and 18 months after surgery （P〈0.05）. Conclusion Knee arthroscopJc PCL reconstruction is effective in the treatment of PCL lacerations. It can improve the knee function and restore the stability of kneejoint, with low incidence of complications and high safety.
