6623例下呼吸道感染儿童9项呼吸道病原体IgM抗体结果分析

Analysis of IgM antibody in 9 respiratory pathogens in 6 623 children

目的:了解本地区下呼吸道感染儿童9项呼吸道病原体的IgM抗体检测结果、流行趋势及其与发病年龄等的相关性,为临床预防及合理诊治提供参考。方法:收集2013年7月至2016年12月19 467例住院儿童呼吸道9项病原学IgM抗体结果进行多维度分析。结果:(1)19 467例下呼吸道感染住院儿童送检6 623例,送检率达34.02%。呼吸道病原体IgM抗体阳性5 784例,阳性率为87.33%。阳性率最高的两种病原体为肺炎支原体(MP,44.05%)和乙型流感病毒(IFB,27.77%)。(2)各种病原体在各个年龄段分布差异有统计学意义(χ~2=455.039,P<0.05):3~5岁年龄组易感MP、嗜肺军团菌(LP)、IFB、肺炎衣原体(CP);<1岁年龄组易感甲型流感病毒(IFA)、呼吸道合胞病毒(RSV);1~2岁年龄组易感腺病毒(ADV);<1岁和3~5岁年龄组易感副流感病毒1、2和3型(PIVS)。结论:本地区近3年来下呼吸道感染患儿主要病原体为MP以及IFB,各病原体易感年龄有差异。

Objective： To investigate the results of Ig M antibody detection of 9 respiratory tract pathogens in children with lower respiratory tract infection in this area,the epidemic trend and its correlation with onset age. Methods： From July 2013 to December 2016,the results of nine items of Ig M antibodies in respiratory tract of 19 467 hospitalized children were collected for multi-dimensional analysis. Results： Among 19 467 inpatients with lower respiratory tract infection,6 623 cases were examined. The positive rate of Ig M antibody in respiratory tract was 5 784 cases. The positive rate was 87. 33. The two pathogens with the highest positive rate were Mycoplasma pneumonia 44. 05%,influenza B virus 27. 77%. There were statistical differences in the distribution of various pathogens among different age groups（ χ~2= 455. 039,P〈0. 05）,the susceptible Mycoplasma pneumoniae（ MPP）,pneumophilic Mycoplasma pneumoniae（ MPM） in the age group of 2-5 years of age were susceptible to pneumonia.Legionella,influenza B virus IFB,Chlamydia pneumoniae,and influenza A virus IFA,respiratory syncytial virus（ RSVV） attack peaked at less than 1 year old. The susceptibility age group of adenovirus ADV is 1-2 years old,and the peak of parainfluenza virus 1,2 and type 3 was 1 year old and 2-5 years old. Conclusion： Mycoplasma pneumoniae and influenza B virus were the most susceptible in children with lower respiratory tract infection caused by virus and atypical respiratory pathogens.
