胎儿纤维连接蛋白定量检测用于预测双胎妊娠早产风险的效果研究 被引量:2

摘要 目的:探讨胎儿纤维连接蛋白(fetal fibronectin,f FN)定量检测用于预测双胎妊娠早产风险的效果研究。方法:选取2016年4月-2017年6月在本院产检并分娩的双胎妊娠的孕妇160例,按照有无腹胀、阴道流血等先兆早产症状,将其分为观察组(无腹胀、阴道流血等先兆早产症状,n=80)和对照组(有腹胀、阴道流血等先兆早产症状,n=80)。两组均进行f FN检测,比较两组f FN早产预测结果;并采用ROC曲线评估f FN定量检测用于预测双胎妊娠早产风险的效果。结果:根据f FN不同检测结果将所有孕产妇分为f FN阳性组54例(观察组22例,对照组32例)和f FN阴性组106例(观察组58例,对照组48例);两组f FN阳性者早产率均高于阴性者,分娩时间均低于阴性者,比较差异均有统计学意义(P<0.05)。两组阳性预测值、阴性预测值、灵敏度、特异度比较,差异均无统计学意义(P>0.05)。结论:f FN定量检测无论在伴随先兆早产症状还是无先兆早产症状的双胎妊娠早产风险中均为有效的预测指标,f FN阴性者早产风险概率较低。 Objective:To explore the effect of quantitative detection of fetal fibronectin(f FN) on predicting the risk of preterm labor in double pregnancy.Method:A total of 160 pregnant women with double pregnancy in our hospital from April 2016 to June 2017 were selected.According to the symptoms of preterm labor,such as abdominal distention and vaginal bleeding,they were divided into observation group(without abdominal distention,vaginal bleeding and other preterm preterm labor symptoms,n=80) and control group(with abdominal distention,vaginal bleeding and other preterm preterm labor symptoms,n=80).They were tested by f FN.The predictive value of f FN in two groups was compared,and the ROC curve was used to evaluate the effect of f FN quantitative detection in predicting the risk of preterm labor in double pregnancy.Result:According to the different test results of f FN,all pregnant and parturition were divided into 54 cases of f FN positive group(22 cases in observation group,32 cases in control group) and 106 cases in f FN negative group(58 cases in observation group and 48 cases in control group).The premature delivery rate of f FN positive pregnant women in two groups were all higher than those of negative pregnant women,and the time of delivery was lower than that of negative pregnant women,the differences were statistically significant(P〈0.05).The positive predictive value,negative predictive value,sensitivity and specificity in two groups were compared,the differences were not statistically significant(P〈0.05).Conclusion:FFN quantitative detection is an effective predictor of risk of preterm delivery in double pregnancy with or without threatened premature labor,and the risk of premature birth of f FN negative people is low.
