非创伤性股骨头坏死不同坏死分期和髋关节功能等级下髋关节积液发生情况

Occurrence of Hip Joint Effusion under Different Stages of Necrosis and Registration of Hip Joint Function in Non Traumatic Necrosis of Femoral Head

目的:对比非创伤性股骨头坏死不同坏死分期和髋关节功能等级下髋关节积液的发生情况,分析髋关节积液与坏死分期和髋关节功能的相关性。方法:以2015年10月-2017年10月本院收治的85例非创伤性股骨头坏死患者作为研究对象,统计髋关节发生积液的情况,同时根据MRI影像评分髋关节积液的等级,与坏死分期和髋关节功能等级进行相关性研究。结果:85例非创伤性股骨头坏死患者共148髋临床资料入选,发生髋关节积液的有130髋,发生率为87.84%。不同性别、不同年龄及不同病程的非创伤性股骨头坏死之间髋关节积液分级分布情况比较,差异均无统计学意义(P>0.05)。坏死分期为Ⅰ级的患者髋关节积液发生率71.43%明显低于Ⅱ级的88.89%、Ⅲ级的100%、Ⅳ级的85.71%,差异有统计学意义(P<0.05),Ⅱ级与Ⅲ级、Ⅲ级与Ⅳ级之间的髋关节积液发生率比较差异均有统计学意义(P<0.05)。髋关节功能等级评分为优、良、中、差的髋关节积液发生率分别为50.00%、88.46%、95.00%及100%,四组之间比较差异均有统计学意义(P<0.05)。结论:非创伤性股骨头坏死患者发生髋关节积液与坏死分期和髋关节功能有一定联系,可参照三者之间的关系评估病情和治疗情况。

Objective：To compare the incidence of hip joint effusion with the stage of necrosis and the function of hip joint by comparing the incidence of hip joint effusion under different stages of necrosis and functional level of hip joint.Method：From October 2015 to October 2017 in our hospital 85 patients with non traumatic necrosis of femoral head patients were selected as the research objects,the statistics of hip joint effusion occurred at the same time,according to MRI image score of hip joint effusion grade and stage of necrosis and hip joint function grade correlation study.Result：85 patients with non traumatic necrosis of femoral head for 148 hips were selected,there were 130 hips in the hydrops of hip joint,and the incidence was 87.84%.There was no significant difference in the distribution of hip effusion among different gender,age and duration of non traumatic femoral head necrosis（P〈0.05）.The incidence rate of hip joint effusion in grade Ⅰ（71.43%） was significantly lower than those in grade Ⅱ（88.89%）,grade Ⅲ（100%） and grade Ⅳ（85.71%）,the difference was statistically significant（P〈0.05）, there were statistical significance in grade Ⅱ and grade Ⅲ,grade Ⅲ and grade Ⅳ（P〈0.05）.The hip functional score for hip joint effusion excellent,good,poor rates were 50.00%,88.46%,95.00% and 100%,the difference among the four groups were statistically significant（P〈0.05）.Conclusion：Hip joint effusion is associated with the stage of necrosis and function of hip joint in patients with non traumatic necrosis of femoral head.We can refer to the relationship among the three to assess the condition and treatment.
