MoCA量表对无症状性脑梗死认知功能障碍的评价及相关危险因素的临床研究 被引量:4

Evaluation of Cognitive Impairment in Asymptomatic Cerebral Infarction with MoCA Scale and Clinical Study of Related Risk Factors

摘要 目的探讨蒙特利尔认知测评(Mo CA)量表对无症状性脑梗死认知功能障碍的评价及相关危险因素。方法随机选取2015年12月—2017年4月期间以头昏、头晕、轻度头痛等非特异性症状来该科住院患者200例,分别进行头颅CT或MRI检查后,将其分为无症状性脑梗死组(SCI组,92例)与非无症状性脑梗死组(非SCI组,108例),同时对其进行Mo CA量表测评,记录Mo CA评分,分析SCI组和非SCI组之间是否存在认知功能障碍的差异。结果 SCI组Mo CA评分[(20.67±4.20)分]明显低于非SCI组[(23.97±3.83)分](t=5.60,P<0.05),SCI组认知功能障碍发生率(89.13%)明显高于非SCI组(49.07%)(χ2=18.17,P<0.05);多发梗死灶、年龄≥65岁及合并高胆固醇血症、高血压等因素,乃是引起或诱发无症状性脑梗死患者发生认知功能障碍的主要危险因素。结论 Mo CA量表具有较高的敏感度,测试快捷简便,可操作性强,可当作评估与筛选脑梗死患者认知功能障碍的良好工具,临床应用价值高。 Objective This paper tries to investigate the evaluation of cognitive impairment in asymptomatic cerebral infarction and related risk factors using the Montreal cognitive assessment(Mo CA) scale. Methods Random select 200 patients with non-specific symptoms such as dizziness, dizzy, and mild headache during December 2015 to April 2017 to undergo head CT or MRI examination and divide them into asymptomatic cerebral infarction group(SCI group, 92 cases) and nonasymptomatic cerebral infarction group(non-SCI group, 108 cases), simultaneously performed Mo CA scale evaluation,recorded Mo CA scores, analyzed whether there was any difference between SCI group and non-SCI group. The difference in dysfunction was analyzed. Results The SCI group Mo CA score[(20.67±4.20)points] was significantly lower than the non-SCI group [(23.97±3.83) points](t=5.60, P〈0.05). The incidence of cognitive impairment in the SCI group was significantly higher(89.13%). In the non-SCI group(49.07%)(χ2=18.17, P〈0.05); multiple infarctions, age≥65 years, combined with hypercholesterolemia, high blood pressure and other factors were caused or induced in patients with asymptomatic cerebral infarction as the major risk factors for cognitive impairment. Conclusion Mo CA scale has high sensitivity, quick and easy testing,and strong operability. It can be used as a good tool for evaluating and screening cognitive impairment in patients with cerebral infarction, and has high clinical application value.
