烟草依赖患者FeNO与肺功能的相关性研究 被引量:6

摘要 目的探讨不同程度烟草依赖患者呼出气一氧化氮与肺功能的相关性。方法收集我院2016年7月至2017年10月呼吸内科门诊及住院烟草依赖患者116例,依据烟草依赖程度分为轻度依赖组48例,中度依赖组36例,重度依赖组32例;以及同期不吸烟对照组28例,分别测定其肺功能(FEV_1%、FEV_1/FVC)和FeNO水平,分析FeNO水平与肺功能(FEV_1%、FEV_1/FVC)的相关性。结果重度烟草依赖组患者的FeNO明显低于轻度烟草依赖组和对照组(P<0.05),轻度烟草依赖组及中度烟草依赖组与对照组FeNO的比较(P>0.05),无统计学意义;轻度、中度和重度烟草依赖组的FEV_1%、FEV_1/FVC均低于对照组(P<0.05),重度烟草依赖组低于中度烟草依赖组(P<0.05),而轻度烟草依赖组与中度烟草依赖组比较则无统计学差异(P>0.05)。轻度、中度及重度烟草依赖组的FeNO与FEV_1%及FEV_1/FVC之间存在一定的正相关性,尤其是以重度烟草依赖组的相关性更为显著(P<0.05)。结论不同程度烟草依赖患者的FeNO与肺功能存在正相关性,烟草依赖越重肺功能损害越重。 Objective To explore the correlation between fraction of exhaled nitric oxide( FeNO) and pulmonary function in different degrees tobacco dependence patients. Methods 116 patients with tobacco dependence from July 2016 and October 2017 were divided into three groups according the degree of tobacco dependence,48 cases in the mild group,36 cases in the moderate group,and 32 cases in the severe group. Another 28 non-smoking subjects were taken as controls( the control group). The FeNO values and pulmonary function indexes were compared among the four groups. The correlation between FeNO and pulmonary function was analyzed with Pearson correlation analysis. Results The FeNO values were significantly lower in the severe group than in the mild group and the control group( 16. 3 ± 9. 2 vs 21. 9 ± 13. 6 and 22. 7 ± 8. 6,P〈0. 05). However,there was no significantly difference between the mild and moderate group and the control group( 21. 9 ± 13. 6 and 19. 4 ± 11. 9 vs 22. 7 ± 8. 6,P〈0. 05). The value of FEV_1% and FEV_1/FVC was significantly lower in the mild,moderate and severe groups than in the control group,and it was significantly lower in the severe group than in the moderate group. However,there was no significantly difference in the value of FEV_1% and FEV_1/FVC between the mild group and the moderate group.There was a positive correlation between FeNO and FEV_1% or FEV_1/FVC in the mild,moderate and severe tobacco dependent groups. In particular,there was a significant positive correlation in the severe tobacco dependent group.Conclusion There is a positive correlation between FeNO and FEV_1% or FEV_1/FVC in the mild,moderate and severe tobacco dependence groups. The heavier the tobacco dependence is,the more serious the lung function damage is.
