乳腺局部点压摄影结合钼靶摄影对乳腺疾病诊断的价值

Value of Breast Ostiole Local Point Pressure Combined with Mammography in Diagnosis of Breast Diseases

目的探讨乳腺局部点压片摄影结合常规钼靶摄影对提高病灶显示率及影像诊断正确率的价值。方法方便收集该院2015年12月—2016年9月72例经过手术治疗的受检者,经仔细触诊后行常规乳腺钼靶摄影(CC和MLO位),分析所获得影像,针对可疑肿块病灶或触诊扪及肿块而未能在影像中完全显示的受检者加摄小孔点压片;将影像诊断结论与手术病理结果的进行对照的分析。结果 72例受检者中,70例病灶表现为肿块型,2例病灶表现为单纯钙化型。常规CC位清晰显示10个病灶、模糊显示47个病灶,遗漏17个病灶;MLO位清晰显示8个病灶、模糊显示59个病灶,遗漏7个病灶;小孔点压清晰显示51个病灶、模糊显示19个病灶;各体位病灶检出率分别为79%、93%、100%。用有序变量的两独立样本比较的秩和检验(Mann-Whitney Test)进行统计分析,CC位于MLO位两种比较(P=0.202>0.05);小孔局部点压分别与CC位、MLO位两两比较(P=0.00<0.05)。结论乳腺数字化钼靶常规摄影结合局部小孔点压摄片,可提高乳腺组织的分辨率,及提高乳腺病灶的检出率,减少误诊和漏诊,为影像诊断及临床治疗提供可靠的影像信息。

Objective To study the value of ostiole local point pressure combined with mammography in diagnosis of breast diseases thus providing basis for the improvement of lesion display rate and imaging diagnosis accurate rate. Methods 72 cases of examined patients in our hospital from December 2015 to September 2016 were convenient selected and treated with CC and MLO after the careful palpation, and the obtained images were analyzed, and the imaging diagnosis conclusions and operative pathological results were compared. Results Of 72 cases of examined patients, 70 cases were featured as the mass type, 2 cases were featured as simple calcification type, and the routine CC showed that 10 lesions were clearly showed, 47 lesions were ambiguously showed, 17 cases were missed, and in the MLO, 8 lesions were clearly showed, 59 lesions were ambiguously showed, 7 lesions were missed, and in ostiole, 51 lesions were clearly showed, 19 lesions were ambiguously showed, and the test rate of lesions in various positions was respectively 79%, 93%, 100%, and the Mann-Whitney Test statistical analysis showed（P=0.2020.005） without statistical significance when CC was locate in MLO position,and the pairwise comparison（P=0.000.05）, when the ostiole local point pressure was respectively located in CC position and MLO position. Conclusion The breast digital molybdenum target routine photography combined with ostiole local point pressure can improve the resolution ratio of breast tissues, improve the test rate of breast lesions, and reduce the missed diagnosis and misdiagnosis, thus providing reliable imaging information for the imaging diagnosis and clinical treatment.
