子午流注纳子法中医定向透药治疗膝痹病的效果及护理

Effect of the iontophoresis with traditional Chinese medicine based on the theory of midnight-noon ebb-flowhour-prescription in the treatment of knee arthralgia and related nursing measures

目的观察基于子午流注纳子法理论择时行中医定向透药治疗膝痹病的效果。方法 60例老年膝痹病患者分为对照组和观察组,各30例。对照组按常规每日行中医定向透药治疗,观察组遵循子午流注理论,选择每日酉时(17:00-19:00)取复溜穴及阴谷穴行中医定向透药治疗。依据中医病证诊断评价疗效,对比2组日本骨科协会(JOA)膝关节骨性关节炎疗效评分和疼痛视觉模拟评分(VAS)。结果治疗1个疗程后,观察组疗效优于对照组(P<0.05)。干预后观察组JOA评分(13.41±1.83)分,高于对照组的(11.07±1.48)分;观察组VAS评分(2.97±1.06)分,低于对照组的(3.53±1.04)分,2组差异有统计学意义(P<0.05)。结论基于子午流注纳子法理论择时行中医定向透药治疗,能有效缓老年患者膝痹症患者疼痛感,改善关节功能。

Objective To observe the effect of iontophoresis with traditional Chinese medicine（TCM） based on the theory of midnight-noon ebb-flowhour-prescription（MNEF） in the treatment of knee arthralgia in senile patients. Methods Totally 60 elderly patients with knee arthralgia were assigned to the control group（ n = 30） and the observation group（ n = 30）. Patients in the control group were treated with routine nursing care and TCMiontophoresis therapy,and those of the observation group were treated with TCMiontophoresis in the period from 5 p. m. to 7 p. m. To evaluate the curative effect of the TCMintervention,and to compare the knee osteoarthritis score of Japanese Orthopedic Association（ JOA） and Visual Analogue Scale（ VAS） on pain between two groups. Results After 1 course of intervention,the curative effect of the observation group was superior to that of controls（ P〈0. 05）. Patients of the observation group achieved a higher knee osteoarthritis outcome score（13. 41 ± 1. 83 vs. 11. 07 ± 1. 48,P〈0. 05）and a lower VAS score（2. 97 ± 1. 06 vs. 3. 53 ± 1. 04,P〈0. 05） compared with that of controls. Conclusion The TCMiontophoresis based on MNEF theory is effective in the treatment of knee arthralgia in senile patients.
