肝脏血管肉瘤的CT表现及鉴别诊断

The CT Manifestations and Differential Diagnosis of Hepatic Angiosarcoma

目的探讨肝脏血管肉瘤的CT征象。方法搜集2005年3月到2017年10月13例（男女比6∶7,平均年龄51.2岁）手术病理证实的肝脏血管肉瘤术前CT资料进行回顾性分析,包括判定主灶、子灶、病灶边界、内部成分、强化方式等。结果位于肝右叶者10例,肝左叶者2例,另有1例肝左、右叶、脾、右心房均发现多发病灶。11例病灶边界不清。增强扫描9例呈不均匀向心性持续性强化,2例无明显强化,2例呈不均匀轻、中度强化。结论大部分肝脏血管肉瘤强化较具特征性,少数分化差的血管肉瘤无强化,分化好的血管肉瘤则需与血管瘤鉴别。

Objective To explore the CT features of hepatic angiosarcoma. Methods 13 cases（ 6 cases were man,7 cases were female,the average age was 51. 2） were diagnosed with hepatic angiosarcoma confirmed by pathology during the study period（ March 2005 to October 2017） were collected,to determine the main focus,sub focus,boundary,internal composition,enhancement patterns et al. Results 10 cases were located in the right lobe of the liver,2 cases located in left hepatic lobe,1 case located in the liver,spleen and right atrium. 11 cases with unclear boundary. Enhanced scan in 9 cases showed inhomogeneous concentric continuous enhancement,2 cases had no obvious enhancement,2 cases showed inhomogeneous mild to moderate strengthen. Conclusion In most of the liver angiosarcomas enhancement is characteristic,a poorly differentiated angiosarcoma without enhancement well differentiated angiosarcoma requires identification from hemangioma.
