摘要
目的检测固醇调节元件结合蛋白1(SREBP1)在多囊卵巢综合征(PCOS)和Ⅰ型子宫内膜癌(EC)患者中的表达情况,探究其与脂代谢之间的关系。方法选取2015年10月至2017年7月在我院妇产科诊治的Ⅰ型EC患者41例、PCOS患者47例为研究对象,选取同期45例正常女性体检者作为对照。对所有受试者行诊刮术进行组织病理学检查,对受试者临床资料和生化参数进行分析。采用酶联免疫吸附(ELISA)法检测血清SREBP1含量,并分析SREBP1含量与PCOS患者和Ⅰ型EC患者脂代谢参数相关性。结果对照组、PCOS组和EC组血清SREBP1水平逐渐升高,两两比较差异均有统计学意义(P<0.05)。Ⅰ型EC组WHR、BMI、TG、TC、LDL-C、HOMA-IR水平高于PCOS组和对照组(P<0.05),HDL-C水平低于PCOS组和对照组(P<0.05);PCOS组WHR、BMI、TG、TC、LDL-C、HOMA-IR水平均高于对照组,HDL-C水平低于对照组(P<0.05)。血清SREBP1含量与PCOS患者BMI、WHR、TG、LDL-C、HOMA-IR水平呈正相关(P<0.05),与Ⅰ型EC患者BMI、WHR、TG、HOMA-IR水平呈正相关(P<0.05)。Logistics多因素回归分析表明,血清SREBP1水平是影响患者PCOS和Ⅰ型EC发病的危险因素(OR=1.327,95%CI 1.025~1.718,P<0.05;OR=2.723,95%CI 1.017~7.219,P<0.05)。结论 SREBP1在PCOS患者和Ⅰ型EC患者血清中含量增加,与PCOS患者和EC患者WHR、BMI、TG、HOMA-IR水平呈正相关,影响PCOS和Ⅰ型EC的发病风险。
Objective To investigate the expression of sterol regulatory element binding protein 1( SREBP1)in polycystic ovary syndrome( PCOS) and type Ⅰ endometrial cancer( EC) and explore its relationship with lipid metabolism.Methods Totally 41 cases of type Ⅰ EC and 47 cases of PCOS were selected from October 2015 to July2017 in the Department of Obstetrics and Gynecology of our hospital, and at the same time, 45 healthy women were selected as control. Histopathological examination was performed on all subjects by curettage, and the clinical data and biochemical parameters were analyzed. Enzyme linked immunosorbent assay( ELISA) was used to detect serum SREBP1 content, and the correlation between SREBP1 content and lipid metabolism parameters in PCOS patients and EC patients was analyzed. Results The levels of serum SREBP1 in control group,PCOS group and EC group were gradually increased,and the differences between each two groups were significant( P〈0. 05); the levels of WHR,BMI,TG,TC,LDL-C and HOMA-IR in EC group were higher than those in PCOS group and control group( P〈0. 05),while the level of HDL-C was lower( P〈0. 05); the levels of WHR,BMI,TG,TC,LDL-C and HOMA-IR in PCOS group were higher than those in control group,while the level of HDL-C was lower( P〈0. 05). Serum SREBP1 content was positively correlated with the levels of BMI,WHR,TG,LDL-C and HOMA-IR in patients with PCOS( P〈0. 05) or the levels of BMI,WHR,TG and HOMA-IR in patients with EC( P〈0. 05). Logistics regression analysis showed that serum SREBP1 level was a risk factor for the incidence of PCOS and EC in patients(OR=1.327,95% CI 1. 025 ~1. 718,P〈0. 05; OR=2.723,95% CI 1. 017 ~ 7. 219,P〈0. 05). Conclusion Serum level of SREBP1 increases in patients with PCOS or type Ⅰ EC, and it is positively correlated with the levels of WHR,BMI,TG and HOMA-IR,which affect the risk of PCOS and type Ⅰ EC.
