摘要
目的探讨CT窗技术在鼻渊辨证分型中的作用。方法观察门诊收治的40例鼻渊患者,通过常规骨窗、软组织窗、肺窗、脑组织窗分别观察鼻窦腔的病变特征,以窦口闭塞、粘膜增厚、窦腔积液,积液性状、皂泡形成,窦腔内钙化等阳性征象为观察指标,并对各指标比较不同窗技术下检出阳性率。结果上颌窦口发生闭塞、粘膜发生增厚、发生钙化占比以骨窗检出最高,脑组织窗观察积液的性状最好,肺窗适用于观察发生皂泡改变,在观察积液有无,上述四个窗无显著差异。结论中医整体观指导下应用骨窗、软组织窗、肺窗、脑组织窗,可以更全面显示病变解剖和中医证候,有利于辨证分型。
Objective To explore the role of CT window technology in the syndrome differentiation of sinusitis.Methods We retrospectively analyzed imaging findings from 40 cases of sinusitis perforation which were confirmed by operation.Utilizing the CT image of the bone window, the abdominal window, the lung window and head windows, we observed traits of the lesions in maxillary sinus. Then, taking the sinus occlusion, thickening of the mucosa, effusion, soap bubble, and calcification as positive signs, we compared the positive rates of the parameters under different windows.Results Occlusion of the maxillary sinus, the thickening and calcification of the mucous membrane were the highest in the bone window. The head window was the best way to observe the character of effusion. The lung window was suitable to observe the change of soap bubble. There was no significant difference between the above four Windows in observation of whether the effusion was observed.
Conclusion Under the guidance of holistic view of TCM, the application of bone window, the abdominal window, lung window, and head window can more fully display the pathological anatomy and clinical status, which is conducive to the syndrome differentiation.
