神经肌肉电刺激对脑卒中后吞咽障碍患者吞咽功能及生活质量的影响 被引量:9

Influence of nerve and muscle electrical stimulation on swallowing function and living quality in stroke patients with dysphagia

摘要 目的:探讨吞咽神经和肌肉电刺激对脑卒中后吞咽障碍患者吞咽功能及生活质量的影响。方法:选取脑卒中后吞咽障碍患者126例作为研究对象,将其随机分为观察组和对照组各63例,对照组给予常规吞咽功能训练,观察组在对照组基础上给予吞咽神经和肌肉电刺激,治疗2周后对两组患者的吞咽功能及生活质量进行比较。结果:与治疗前比较,治疗后两组患者洼田饮水试验评分均有不同程度下降,且观察组明显低于对照组(P<0.05);两组患者电视透视检查(VFSS)评分及总分均有不同程度的升高,且观察组明显高于对照组(P<0.05);两组患者生存质量各个维度评分及总分均有不同程度升高,且观察组明显高于对照组(P<0.05);观察组患者治疗总有效率高于对照组(P<0.05)。结论:吞咽神经和肌肉电刺激可有效促进脑卒中吞咽功能障碍患者吞咽功能恢复,提高其生活质量。 Objective: To study the influence of nerve and muscle electrical stimulation swallowing function and living quality in stroke patients with dysphagia. Methods: 126 cases of stroke patients with dysphagia were selected as the study objects, and were divided into observation group and control group randomly, 63 cases in each group. The control group were given routine swallowing training, and the observation group were given swallowing nerve and muscle electrical stim- ulation on the basis of the control group. Clinical effects were observed in the two groups. Results: Compared with that before treatment, the scores of drinking water test decreased in different degrees of the two groups, which of the observa- tion group were significantly lower than those in the control group (P〈0.05). Scores of appetite, eating time, food selection, swallowing burden, mental health, diet, fear of swallowing symptoms, physical function, fatigue and sleep scores of each dimension increased to varying degrees, which of the observation group were significantly higher than those of the control group (P〈0.05). The total effective rate of the observation group was 96.83%, significantly higher than that of 85.71% of the control group (P〈0.05). Conclusion: Neuromuscular electrical stimulation combined with swallowing training can ef- fectively promote the recovery of swallowing function in patients with dysphagia after stroke, with significant clinical effect.
