肝硬化出血患者营养状况分析及早期肠内营养护理 被引量:10

摘要 目的评估肝硬化并发上消化道出血患者营养状况,探讨消化内镜止血后早期肠内营养(EN)支持护理的疗效。方法纳入肝硬化并发上消化道出血患者95例,根据NRS2002营养风险评分量表评估患者营养状况。患者行消化内镜止血后,随机分为两组,对照组47例采用常规护理;营养组48例在术后12 h内给予肠内营养支持治疗及护理。治疗2 w后,比较两组营养状况、再出血及并发症发生率。结果两组入组时普遍存在营养不良,70.5%的患者NRS2002≧3分。治疗2 w后,营养组营养状态明显改善,ALB、TP水平均较治疗前明显提高(P<0.05),且显著高于对照组(P<0.05)。两组治疗后BMI指数无明显差异(P>0.05)。营养组平均住院(15.8±4.2)d,显著短于对照组的(18.3±4.6)d(P<0.05)。两组治疗期间内腹泻、恶心呕吐、肺部感染、胃储留等并发症的发生率无明显差异(P>0.05),但营养组持续性腹水及再出血发生率较对照组低(P<0.05)。结论肝硬化并发上消化道出血患者营养不良发生率高,早期肠内营养支持治疗和护理能改善患者营养状况,降低再出血发生率,促进患者康复。
