胰岛素泵治疗妊娠期糖尿病的临床疗效及对妊娠结局的影响

Efficacy of insulin pump and its impact on the pregnancy outcome for gestational diabetes mellitus patients

目的探讨妊娠期糖尿病采取胰岛素泵给药的临床疗效及对妊娠结局的影响。方法选取接收的82例妊娠期糖尿病患者,入院时间为2016年3月~2017年3月期间,随机分为对照组(n=41,采取多次皮下注射胰岛素)与观察组(n=41,采取胰岛素泵持续给药),观察比较两组的临床疗效及妊娠结局。结果治疗后,两组HbAlc及2 hPG、FPG水平均显著下降,而观察组较对照组下降更明显(P<0.05),且观察组血糖达标时间较对照组显著缩短(P<0.05)。观察组患者妊娠期高血压、新生儿低血糖、胎膜早破、巨大儿等发生率较对照组显著低(P<0.05)。结论胰岛素泵持续给药控制血糖效果显著,从而有效改善患者妊娠结局。

Objective To discuss the clinical efficacy of insulin pump and its impact on the pregnancy outcome for gestational diabetes mellitus （GDM） patients. Methods 82 GDM patients treated from March 2016 to March 2017 in our hospital were selected. The subjects were randomly assigned to control group and observation group, 41 cases in each group. The patients took the repeated subcutaneous injection of insulin or continuous pump of insulin respectively. The clinical efficacy was considered. Results After treatment, the index of HbAlc （glycosylated hemoglobin）, 2hPG（2h-postprandial plasma glucose） and FPG （fasting plasma glucose）for observation group were even lower than those of the control group （P 〈0.05）; the time of reaching the standard blood glucose for observation group was significantly shorter than control group （P 〈0.05）; the incidence rate of pregnancy-induced hypertension, hypoglycemia, premature rupture of fetal membranes and fetal macrosomia for observation group were significantly lower than those of control group （P 〈 0.05）.Conclusion The continuous pump of insulin can better control the blood glucose, and improve the pregnancy outcome.
