回馈教学法对脑胶质瘤术后肢体运动障碍患者康复训练的影响 被引量:14

Effect of feedback teaching method on rehabilitation training of patients with limb movement disorder after brain glioma surgery

摘要 目的探讨回馈教学法在脑胶质瘤术后肢体运动障碍患者康复训练中的应用效果。方法选取神经外科病房收治的功能区脑胶质瘤患者为研究对象,按人院时间顺序将2016年7-8月入选的30例患者为对照组,2016年9-11月人选的30例患者为观察组。对照组采用神经外科术后常规护理与功能锻炼指导,观察组在对照组基础上给予回馈教学法。分别于患者术后第3天、术后2周末、术后4周末,评估2组患者的四肢肌力情况、日常生活活动能力及满意度。结果观察组患者在术后4周末双下肢的肌力恢复情况优于对照组(t=5.015、5.320,P〈0.05);术后2周末和术后4周末观察组患者日常生活能力得分高于对照组(t=5.360、6.510,P〈0.05);2组患者日常生活能力得分不同时间点比较差异有统计学意义(F=68.64、65.07,P〈0.01);观察组患者日常生活能力得分高于对照组(t=2-315,P〈0.05);术后2周末和术后4周末观察组患者满意度高于对照组(t=2.042、4.317,P〈O.05)。结论对脑胶质瘤术后肢体运动障碍患者,康复训练时采用回馈教学法,可促进患者肌力恢复,尤其是双下肢肌力恢复,改善患者日常生活能力,提高患者满意度。 Objective To explore the application effect of video teaching combined with Teach- back method in rehabilitation training of patients with hemiplegia. Methods The patients with glioma in eloquent area in department of neurosurgery were selected as the research subjects, grouped by time sequence of admission. Between July 2016 to August 2016, 30 patients were selected as control group. Between September 2016 to November 2016, 30 patients were selected as observation group. The control group using conventional department of neurosnrgery postoperative routine nursing and hemiplegia rehabilitation education. The observation group was given feedback teaching method on basis of the control group. On the third days after operation, 2 weeks after operation and 4 weeks after operation respectively, the muscle strength, activities of daily living and satisfaction of two groups were evaluated. Results The recovery of muscle strength of the lower limbs of the observation group was better than the control group at 4 weeks after operation (t=5.015, 5.320, P 〈 0.05); At 2 weeks after operation and 4 weeks after operation, the daily living ability score of the observation group was higher than the control group (t= 5.360, 6.510, P 〈 0.05); There were statistically significant differences in the scores of daily living ability between the two groups at different time points (F=68.64, 65.07, P〈 0.01); The observation group had higher scores of daily living ability than the control group (t=2.315, P 〈 0.05); At 2 weeks after operation and 4 weeks after operation, the patient satisfaction of the observation group was higher than the control group (t=2.042, 4.317, P 〈 0.05). Conclusion For glioma patients with postoperative limb movement disorders, feedback teaching method combined with rehabilitation training can promote the recovery of muscle strength, especially the lower limbs, improve the ability of daily living and the satisfaction of patients.
