营养不良与慢性阻塞性肺疾病的临床分析 被引量:3

THE CLINICAL SIGNIFICANCE OF MALNUTRITION IN COPD PATIENTS

摘要 目的探讨营养不良与慢性阻塞性肺疾病(COPD)患者病情之间的相关性研究及其临床意义。方法选择2015年12月—2017年12月于华北理工大学附属医院呼吸内科住院的100例慢性阻塞性肺疾病急性加重期患者,根据测定其体重指数(BMI),血液检查中的淋巴细胞计数、血清白蛋白、血清球蛋白指标,分析其与病情之间的临床意义。结果依据肺功能评估后分成四组,四组间营养指标行方差分析提示:组间白蛋白、BMI和淋巴细胞计数差异有统计学意义(P<0.05),球蛋白计数差异无统计学意义(P>0.05);对急性加重期高风险组以及低风险组间指标进行t检验,高风险组中BMI、白蛋白、淋巴细胞计数四组数值均低于低风险组,差异有统计学意义(P<0.05)。结论 BMI、白蛋白和淋巴细胞计数等营养指标可用于COPD患者营养评估;营养不良的存在可增加COPD急性加重风险,加快肺功能的下降;临床工作中应重视COPD患者的营养问题,及时实施营养干预。 Objective To investigate the correlation between malnutrition and the condition of COPD patients and its clinical significance.Methods From December 2015 to December 2017,100 cases of patients with acute exacerbation of chronic obstructive pulmonary disease diagnosed in author' s hospital were selected as the research subjects.Body mass index (BMI) ,lymphocyte count, serum albumin and serum globulin were analyzed. Results The patients were divided into four groups according to pulmonary function assessment.Nutrition index showed that the differences among the four groups were statistically significant on BMI, albumin and lymphocyte count (P〈0.05), but globulin count had no statistically significant (P 〉0.05).In acute exacerbation of high risk group,BMI, albumin and lymphocyte count were lower than the low risk group, the difference was statistically significant (t-test,P〈0.05).Conelusion BMI, albumin and lymphocyte count and other nutri- tional indicators can be used to evaluate the nutrition in patients with COPD; malnutrition can increase the risk of acute exacerbation of COPD,accelerate the decline in lung function.We should pay attention to nutritional problems of COPD patients in the clinical work,and implement timely nutritional intervention.
