老年桡骨远端AO-C型骨折的手术选择

SURGICAL OPTIONS FOR AO TYPE C DISTAL RADIUS FRACTURES IN THE ELDERLY

目的比较外固定架固定术与切开复位内固定术两种手术方式的临床处理指标和手术效果。方法回顾性分析2013年6月—2016年6月华北理工大学附属医院骨外科收治的41例AO-C型桡骨远端骨折患者,其中19例行外固定架固定结合有限切开内固定术,22例行切开复位内固定术,外固定架组在手术时间、手术出血量、总住院时间及骨折愈合时间上均优于内固定组,而在关节面移位、桡骨短缩、掌倾角和尺偏角的恢复程度方面内固定组表现更佳,Cooney评分及手术并发症发生率差异无统计学意义。结论可根据患者的功能期望值选择不同的手术方式。

Objective To compare the clinical treatment index and operation effect of external fixation and open reduetion＆internal fixation.Methods Forty - one patients with AO type C distal radius fracture were analyzed retrospectively from June 2013 to June 2016 in author＇s hospitaLAmong them,19 patients were trea- ted with external fixation,and 21 patients were treated with open reduetion＆internal fixation. Results Patients treated with external fixation performed better than those treated with open reduction＆internal fixation in operation time,bleeding amount, total hospital stay and fracture healing time, but less better in displacement of articular surface, shortening of radius ,obliquity and ulnar deviation angle.The two groups performed no significant difference in Cooney scores and complication rates.Conclusion Different operations can be chosen according to patients＇ functional expectation.
