某医院血标本中病原菌分布及耐药性分析

摘要 目的了解医院血标本分离的病原菌分布和耐药性变迁,指导临床合理使用抗菌药物。方法采用细菌分离鉴定和药敏试验方法,对某医院临床采集的血标本进行检测与分析。结果从6 857份血标本中共检出病原菌672株,阳性率为9.8%。病原菌以大肠埃希菌、凝固酶阴性葡萄球菌、肺炎克雷伯菌和金黄色葡萄球菌为主。临床分离的优势菌普遍耐药,革兰阳性菌仅对万古霉素和利奈唑胺敏感,大肠埃希菌和肺炎克雷伯菌仅对碳青霉烯类抗菌药物敏感。结论该医院临床采集的血标本分离的病原菌以革兰阴性菌为主,耐药形势严峻,应以药敏试验结果作为选用抗菌药物的依据。 Objective To investigate the distribution and drug resistance of clinical isolates of blood culture from hospital blood samples and to provide evidences for clinical rational drug uses. Methods The blood samples collected from a hospi- tal were detected and analyzed by means of bacterial isolation identification and drug sensitivity test. Results Totally 672 strains of pathogens were isolated from 6 857 blood culture samples, with the positive rate of 9.8%. Strains of Escherichia coli ,eoagulase negative Staphylococcus, Klebsiella pneumoniae, Staphylococcus aureus were the main pathogens that ranking the first four isolated strains. The dominant bacteria isolated clinically were generally resistant to drugs. The Gram - positive bacteria were only sensitive to Vancomycin and Linazolamine. Escherichia coli and klebsiella pneumoniae were only sensitive to Carbapenems. Conclusion The pathogenic bacteria isolated from the blood samples collected in this hospital are mainly gram -negative bacteria, and the situation of drug resistance is severe. Results of drug sensitivity test should be used as the basis for choosing antibacterial drugs.
