生长抑素联合鼻胃管胃肠减压治疗腹部手术后早期炎性肠梗阻的效果分析

Somatostatin Combined with Gastrointestinal Decompression via Nasogastric Tube in the Treatment of Early Postoperative Inflammatory Small Bowel Obstruction of Abdominal Surgery:Efficacy Analysis

为探讨生长抑素皮下注射联合鼻胃管胃肠减压治疗腹部手术后早期炎性肠梗阻的临床疗效,将76例腹部手术后早期炎性肠梗阻患者随机均分为观察组和对照组,各38例,对照组予鼻胃管胃肠减压治疗,观察组在对照组处理基础上予生长抑素皮下注射治疗,对比2组治疗效果。结果显示,观察组腹围缩小、胃肠减压量显著多于对照组,P<0.05;观察组呕吐停止时间、腹痛消失时间、排便恢复时间、气液平面消失时间显著短于对照组,P<0.05。2组治疗后C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平显著低于治疗前,P<0.05;其中观察组治疗后CRP、TNF-α水平更低,P<0.05。观察组并发症发生率显著低于对照组,P<0.05。结果表明,生长抑素皮下注射联合鼻胃管胃肠减压能够有效减轻腹部手术后早期炎性肠梗阻患者的临床症状,减轻炎症反应,并发症少,具有临床推广使用价值。

This study was to investigate the clinical effect of somatostatin subcutaneous injection combined with gastrointesinal decompression via nasogastrie tube for treating early postoperative inflammatory small bowel obstruction（EPISBO）,randomly divided 76 cases of the EPISBO into observation group and control one, 38 cases for each, which the latter received gastrointestinal decompression via nasogastric tube, on this basis the former was added somatostatin subcutaneous injection; then compared both group＇s efficacy of treatment.As results,in the former patient＇s abdominal perimeter decreased,their gastrointestinal decom pression volume was significantly more than that of the latter＇s patients（ P 〈0.05）;the time of vomiting stopped,of abdomen pain disappeared, of defecation recovered, and of gas-liquid level disappeared in the former were all shorter than that in the latter（ P 〈0.05）;after treatment the levels of C reactive protein （CRP） ,and of TNF-α were significantly lower than before treatment（ P 〈0.05） ,but in the former the levels of both were more lower（ P 〈0.05）;what＇s more, the complication incidence of the former was significantly lower than that of the latter （ P 〈0.05）.Results show that somatostatin subcutaneous injection plus gastrointestinal decompression via nasogastric tube can effectively allievate the clinical symptoms and inflammatory response of EISBO patients after abdominal surgery, and its complication less occured. It is worthy of clinical popularization and application.
