28例特殊部位肝癌微波消融的治疗策略和远期疗效 被引量:1

摘要 目的评价打水、断流等特殊方法对特殊部位肝癌行微波消融治疗的可行性和远期疗效。方法 28例特殊部位肝癌患者,毗邻重要器官(如膈肌、胆囊、胃肠等)的病灶19例,突出肝表面9例,均行人工胸腹水和(或)断流等技术后行微波消融治疗。结果 19例毗邻重要器官的病灶,全部成功行人工胸、腹水,病灶显示清楚或肝脏与毗邻器官之间充分分离。注入的液体量400-2 500(1 160±500)ml,9例突出肝表面的病灶全部行断流术。所有病灶消融完全,胸、腹水在1~5 d内排出或吸收,无一例发生严重并发症。28例术后随访12-24个月,CT或MIR增强检查证实所有患者均未发生病灶残留、复发等情况。结论联合人工胸腹水和(或)断流等技术行微波消融治疗,优化消融针的进针路径,拓宽消融治疗的适应证,使消融治疗更加准确、安全、有效。
