炎症性肠病患者IgG4表达水平与疾病活动度的相关性研究

Relativity Study on IgG4 Expression Level in IBD Patients and the Disease Activity Degree

为了探讨炎症性肠病(IBD)患者机体免疫球蛋白G4(IgG4)表达水平与疾病活动度的相关性,将2013年3月至2017年11月甘肃省徽县人民医院收治的30例IBD患者设为研究组,将同时期的30名健康体检者设为对照组。采用免疫散射比浊法、免疫组织化学法对比IgG4浓度与疾病活动度的相关性。结果显示,溃疡性结肠炎(UC)和克罗恩病(CD)患者血清IgG4浓度均<1.35 g/L;急性期UC、CD的IgG4阳性浆细胞浸润数较高,均显著高于缓解期及健康体检者(P<0.05);UC、CD患者机体内IgG4阳性浆细胞浸润数与Mayo评分呈现显著正相关关系(r=0.362、r=0.293,P<0.001、P<0.001)。结果表明,炎症性肠病患者血清IgG4表达水平与疾病活动度呈正相关,临床上可将IgG4表达水平作为诊治炎症性肠病的评价指标之一,具有一定的临床意义。

This study was to explore the relativeity of IgG4 expression level in IBD（inflammatory bowel disease） patients and their disease activity degree（DAD）, enrolled 300 IBD patients who admitted and re- ceived treatment in the leater author＇s hospital from March 2013 to Nov. 2017 yr. as research group,at same time 30 health examinee as control one,adopted immuno-nephelometry and immuno-his-chemistry to contrast the relativity of IgG4 level and DAD.As results,in ulcerative colitis （UC） patients and in Crohn＇s diseases（CD） patients their serum level of IgG4 were all less 1.35 g/L,and in acute UC and CD patients the number of positive plasma cells infiltrated was higher and significantly higher than that in catabasis pa- tients and health examinee（ P 〈0.05）;in UC and CD patients the significant proprotional＇relationship be- tween positive plasma cells infiltrated and Mayo score ratings stood up（ r = 0.362, r = 0.293, all P 〈 0.01）.Results show that in IBD patients the relativity of serum IgG4 level and DAD appeares proportional, thus clinically IgG4 level could be taken an evaluation index for diagnosing ＆ treating IBD,i.e which had certain clinical significance.
