子宫内膜切除术治疗功能性子宫出血的应用效果及栓塞发生情况比较

Application of endometrial resection in the treatment of functional uterine bleeding and comparison of embolization

目的探讨功能性子宫出血应用子宫内膜切除术(EA)的临床意义。方法以2015年8月至2016年11月本院功能性子宫出血患者72例为研究对象,遵循完全随机原则,均分为2组各36例。对照组采取腹式子宫次全切除术,观察组采取EA。观察2组手术、贫血纠正及月经改善情况。结果观察组操作时间(95.28±5.33)min、失血量(82.85±9.03)ml、下床活动时间(20.33±4.82)h、住院时间(3.82±0.97)d、贫血纠正概率(94.44%)、月经改善概率(88.89%),与对照组比较差异均具有统计学意义(P>0.05)。2组患者术中、术后均无血栓栓塞等严重手术并发症情况发生。但观察组患者有2例发生轻微下腹部疼痛;对照组5例出现下腹部疼痛。组间比较,无统计学差异(χ~2=0.633,P=0.426)。结论 EA应用于功能性子宫出血治疗中,临床价值较高。

Objective To explore the clinical significance of endometrial ablation for functional uterine bleeding. Methods 72 patients with functional uterine bleeding in our hospital from August 2015 to November 2016 were selected as study subjects and followed the principle of complete randomization. All patients were divided into two groups and 36 cases. The control group received abdominal hysterectomy and the observation group took endometrial resection. Observed the 2 groups of surgery, anemia correction and menstrual improvement. Results The operating time of the observation group was（95.28±5.33） min, blood loss（82.85±9.03） ml, ambulation time（20.33±4.82）h, length of hospital stay（3.82±0.97） d, probability of anemia correction（94.44%）, menstruation The probability of improvement（88.89%） was significantly different from that of the control group（P〈0.05）. There were no serious complications such as thromboembolism in the two groups during and after surgery. However, in the observation group, there were 2 cases of minor lower abdominal pain; in the control group, 5 cases had lower abdominal pain. There was no statistical difference between groups（χ2=0.633, P=0.426）. Conclusion Endometrial resection is a clinically valuable method for the treatment of functional uterine bleeding.
