血清CRP、CD64、PCT在小儿肺炎鉴别诊断中的临床研究 被引量:2

摘要 目的 分析CRP、CD64和PCT在小儿肺炎鉴别诊断中的应用。 方法 搜集该院自2016年1月至2018年5月就诊的小儿肺炎患者120例作为肺炎组,同时选取该阶段健康儿童共50例作为对照组,分别采集研究对象的空腹静脉血液标本送检,测定血清CRP、CD64、PCT水平,并采集肺炎患者的咽拭子标本,测定肺炎感染病原体类型,按照病原体类型分为细菌组(细菌性肺炎)和非细菌组,比较各组的血清CRP、CD64、PCT,分析诊断的准确度。 结果 肺炎组患者的血清CRP、CD64、PCT值都比对照组明显增加( P 〈0.05);细菌组患者的CRP、CD64、PCT值都比非细菌组明显增加( P 〈0.05);三者联合诊断的特异性是91.94%,敏感性是 93.10 %,明显高于单一诊断( P 〈0.05)。 结论 CRP、CD64、PCT可用于鉴别诊断小儿细菌性肺炎,三者联合诊断的准确度较高。 Objective: To analyze the effect of CRP,CD64 and PCT on differential diagnosis of pneumonia in children. Methods : 120 cases with pneumonia in our hospital from January 2016 to May 2018 were collected as pneumonia group.At the same time,50 healthy were selected as control group.The fasting venous blood were collected,and serum levels of CRP,CD64 and PCT were measured.Pharynx swab of patients with pneumonia were collected to determine pneumonia.Patients were divided into bacterial group (bacterial pneumonia) and non bacterial group by pathogen type.CRP,CD64 and PCT were compared in each group,and accuracy of diagnosis was analyzed. Results: CRP,CD64 and PCT values in pneumonia group were significantly higher than those in control group ( P 〈0.05),and CRP,CD64 and PCT values in bacterial group were significantly higher than those in non bacterial group ( P 〈0.05).Specificity of combined was 91.94%,sensitivity was 93.10%,and which were higher than single diagnosis( P 〈0.05). Conclusion CRP,CD64 and PCT can be used for differential diagnosis of bacterial pneumonia in children.The accuracy of combined diagnosis is better.
