探讨超微血管成像技术在评价颈动脉斑块内新生血管的应用

摘要 目的探讨超微血管成像(SMI)技术在评价颈动脉斑块内新生血管的应用的可行性.方法选取经二维常规超声检出有低回声、等回声及以中低回声为主的混合性回声斑块且厚度≥2.0mm的41例患者(斑块52例),据其是否使用过有他汀类药物,分为用药组与非用药组,并对其进行SMI技术及超声造影(CEUS)的检测,根据分级标准进行分级,评价两种检测方法的分级结果的一致性.CEUS利用时间G强度曲线定量分析斑块的峰值强度(PIt)与颈动脉血管内的峰值强度(PIr)的比值(PIt/PIr)与SMI分级之间的相关性.结果SMI、CEUS两种检测方法分级结果一致性较好(kappa=0.700〉0.4).对各斑块的SMI分级与CEUSPIt/PIr比值成正相关,相关性显著,相关系数为0.403(P〈0.01);对各斑块的CEUS分级与斑块CEUS的PIt/PIr成正相关,相关性显著,相关系数为0.434(P〈0.01);CEUS分级结果用药组与非用药组分级结果差异有统计学意义(P=0.034〈0.05);超微血管成像(SMI)技术分级结果用药组与非用药组超声造影分级结果差异有统计学意义(P=0.047〈0.05).结论SMI技术可以作为检测斑块内新生血管的首选方法.
