腹部彩超与经阴道彩超在子宫内膜癌病变诊断中的价值

Significance of abdominal color Doppler ultrasound and transvaginal color Doppler ultrasound in the diagnosis of endometrial carcinoma

目的探讨腹部彩超与经阴道彩超在子宫内膜癌病变诊断中的应用价值。方法选取2016年4月至2017年4月间海南省第三人民医院收治的40例子宫内膜癌病变患者纳入子宫内膜癌病变组,均经手术病理证实,均采用经腹部彩超与经阴道彩超检查,统计两种检测方法的子宫内膜癌病变诊断符合率与相关图像清晰度评分。并纳入同期健康体检者40例女性为对照组,实施经阴道检查,比较两组研究对象子宫动脉及相应病灶的血流情况。结果与病理诊断结果比较,经阴道诊断的诊断符合率为85.0%(34例),高于经腹部诊断组的55.0%(22例),差异有统计学意义(P<0.05)。经阴道超声诊断下的图像清晰程度评估总优良率为92.5%,高于经腹部超声诊断的75.0%,差异有统计学意义(P<0.05)。经阴道超声诊断的图像清晰程度评分为(4.85±0.11)分,高于经腹部超声诊断的(3.71±0.12)分,差异有统计学意义(P<0.05)。子宫内膜癌病变组患者血流动力指数值低于健康对照组,血流阻力指数值高于健康对照组,两组比较,差异均有统计学意义(均P<0.05)。结论与经腹部彩超检查比较,经阴道彩超在子宫内膜癌病变临床诊断中可获得更为理想的诊断效果,具有较高的应用价值。

Objective To analyze the value of abdominal color Doppler ultrasound and transvaginal color Doppler ultrasound in the diagnosis of endometrial carcinoma. Methods A total of 40 patients with endometrial cancer lesions treated at The Third People＇s Hospital of Hainan Province from April 2016 to April 2017 were selected. All the patients were confirmed by clinical pathology,and underwent abdominal ultrasound and transvaginal ultrasound examination. Accordance rate of the two detection methods for endometrial cancer lesions and related image clarity score were calculated. In addition,40 women who received physical examination at the hospital during the same period were selected as the control group. Vaginal blood examination was performed on these women,and the blood flow of uterine artery and corresponding lesions were compared between the two groups. Results Compared with pathological diagnosis results,the diagnostic accordance rate of endometrial cancer was 85%（ 34 patients）,which was significantly higher than 55. 0%（ 22 patients） of the transabdominal diagnostic group（ P〈0. 05）. The overall excellent and good rate was 92. 5% for the images obtained by vaginal diagnosis which was significantly higher than 75%of the abdominal diagnosis（ P〈0. 05）. The score of image clarity was（ 4. 85 ± 0. 11） for the transvaginal diagnostic group,which was significantly higher than（ 3. 71 ± 0. 12） of abdominal diagnosis,（ P〈0. 05）.The blood flow RI value was significantly higher in the endometrial carcinoma group than in the healthy control group,and the PI value was significantly lower than in the healthy control group（ all P〈0. 05）.Conclusion Transvaginal color Doppler ultrasound in the diagnosis of endometrial cancer can get a more ideal diagnosis efficacy than abdominal color Doppler ultrasound with a high application value.
