预知护理干预对胃癌患者术后效果及肠道功能的影响

Effect of predictive nursing intervention on intestinal function and quality of life in patients with gastric cancer after operation

目的探讨预知护理对胃癌患者术后康复效果及肠道功能的影响。方法选取2017年1月至2018年1月间汉中市人民医院收治的120例行胃癌切除术的胃癌患者,采用随机数表法分为观察组和对照组,每组60例。观察组患者采用预知护理干预,对照组患者采用传统常规护理,比较两组患者的情绪和护理满意程度、术后并发症及肠道功能等表现。结果观察组患者术后肠鸣音恢复时间、肛门排气时间、饮食恢复时间、排便周期及住院时间均少于对照组,两组比较,差异均有统计学意义(均P<0.05)。两组患者术后均发生胃溃疡、吻合口瘘或切口感染、肺部感染、肠梗阻、呕吐及发热等并发症,观察组患者并发症发生率为13.3%,对照组为31.7%,两组比较,差异有统计学意义(P<0.05)。观察组患者术后生活质量各项评分均高于对照组,两组比较,差异均有统计学意义(均P<0.05)。观察组患者的焦虑和抑郁评分均低于对照组,两组比较,差异均有统计学意义(均P<0.05)。观察组患者护理满意度为96.7%,高于对照组的73.3%,两组比较,差异有统计学意义(P<0.05)。结论胃癌患者术后采用预知护理干预,可增强患者的生存信心,促进肠道生理功能改善,辅助良好的身心状态,促进其预后改善。

Objective To study the effect of pre-known nursing on the rehabilitation and function of the intestine in the patients undergoing the operation for gastric cancer. Methods From January 2017 to January 2018,120 patients undergoing resection for gastric cancer were selected at Hanzhong People＇s Hospital and Baoji Hospital of Traditional Chinese Medicine were randomly divided into an observation group and a control group with 120 patients in each group. The observation group received pre-known nursing intervention and the control group recived traditional routine nursing. Patients＇ emotion,patients＇ satisfaction with nursing and postoperative complication and intestinal function were compared between the two groups.Results After the operation,the recovery time of the bowel sound,the time of anus exhaust,the time of diet recovery,the cycle of defecation and the time of hospitalization were less in the observation group than in the control group（ all P〈0. 05）. Complications including gastric ulcer,anastomotic fistula or wound infection,pulmonary infection,intestinal obstruction and fever occurred in both groups. The incidence of complications was 13. 3% for the observation group and 31. 7% for the control group（ P〈0. 05）. The postoperative quality of life score was higher in the observation group than in the control group（ P〈0. 05）. The scores of anxiety and depressio were lower in the observation group than in the control group（ P〈0. 05）.The nursing satisfaction rtw was 96. 7% for the observation group which was,higher than 73. 3% of the control group（ P〈0. 05）. Conclusion Predictive nursing intervention can play a significant role in the intervention,which enhances the patients＇ confidence in survival,promote the improvement of the physiological function of intestine,play a good auxiliary role in the physical and mental state,and promote the improvement of the prognosis in patients receiving the operation for gastric cancer.
