疏肝清胃方联合西药治疗幽门螺杆菌阳性慢性非萎缩性胃炎的临床观察

Clinical Observation on Treatment of Helicobacter Pylori Positive Chronic Non-Atrophic Gastritis with Shuganqingwei Prescription Combined with Western Medicine

目的:观察疏肝清胃方联合西药治疗幽门螺杆菌(Hp)阳性慢性非萎缩性胃炎的临床疗效。方法:将广州中医药大学深圳医院及广东省第二中医院2016年9月至2018年2月82例Hp阳性慢性非萎缩性胃炎患者随机分为观察组28例,对照1组、对照2组各27例,观察组采用疏肝清胃方联合四联抗Hp方案(雷贝拉唑肠溶片+枸橼酸铋钾颗粒+克拉霉素缓释片+阿莫西林胶囊)口服;对照1组予疏肝清胃方煎剂口服;对照2组予上述四联西药口服。14 d后观察组停用2联抗生素剂及铋剂,以疏肝清胃方+雷贝拉唑钠肠溶片10 mg,每日1次口服,对照2组以雷贝拉唑钠肠溶片10 mg,每日1次口服,对照1组药物不变,继续治疗14 d。结果:三组患者症状改善情况比较,差异无统计学意义(P>0.05),观察组内镜疗效优于对照1组、2组,差异具有统计学意义(P<0.05);在Hp根除率方面,观察组显著高于对照1组、2组,差异具有统计学意义(P<0.05)。结论:中药疏肝清胃方在改善症状方面与西药相当,其联合西药对Hp阳性慢性非萎缩性胃炎的治疗具有显著疗效。

Objective To observe the clinical efficacy of Shuganqingwei prescription combined with Western medicines for the treatment of Helicobacter pylori（Hp） positive chronic non-atrophic gastritis. Methods 85 patients with Hp-positive chronic non-atrophic gastritis seleced in Guangzhou University of Chinese Medicine Shenzhen Hospital and Guangdong Provincial second Chinese Medicine Hospital from September 2016 to February 2018 were randomly assigned to the treatment group including 28 patients,or control group 1 or control group 2, 27 patients each; the treatment group received Shuganqingwei prescription combined with the quadruple anti-Hp regimen（rabeprazole enteric-coated tablets＋potassium citrate tablets＋clarithromycin sustained-release tablets＋amoxicillin capsules for 28 days）; control group 1 received Shuganqingwei prescription alone for 28 days;control group 2 was initiated on the above quadruple regimen,which was continued for 14 days before discontinuation of potassium citrate tablets,clarithromycin sustained-release tablets and amoxicillin capsules and switch to Shuganqingwei prescription ＋ rabeprazole enteric-coated tablets 10mg QD for 14 days. Results There were no significant differences across the three groups in symptoms improvement（P〉0.05）; the treatment group was superior over control groups 1 and 2 in endoscopic efficacy（P〈0.05）; the Hp eradication rate was significantly higher in the treatment group than in control groups 1 and 2（P〈0.05）. Conclusion The traditional Chinese medicine Shuganqingwei prescription is equivalent to western medicine in terms of improving symptoms, and the combination of western medicine has a significant effect on the treatment of Hp-positive chronic non-atrophic gastritis.
