子宫内膜异位症应用米非司酮治疗的效果及作用机制分析

Analysis of the effect and mechanism of mifepristone in the treatment of endometriosis

目的研究与分析子宫内膜异位症应用米非司酮治疗的效果及作用机制。方法选取我院收治的子宫内膜异位症患者100例为对象进行研究,利用电脑随机法分为对照组和观察组,各50例。对照组采用丹那唑于月经第1d开始口服治疗,而观察组采用米非司酮于月经第1d开始口服治疗。观察比较两组治疗前后血清CA125(血清癌抗原125)水平变化、临床疗效、妊娠率、复发率以及不良反应情况。结果两组治疗前血清CA125水平对比差异不显著,且P>0.05;而治疗后观察组血清CA125水平与对照组相比下降更为显著,且P<0.05。观察组治疗有效率为94.0%(47/50)与对照组80.0%(40/50)相比差异有统计学意义,且P<0.05。观察组妊娠率为54.0%(27/50)、复发率15.0%(15/50)与对照组40.0%(20/50)、14.0%(7/50)相比差异有统计学意义,且P<0.05。观察组不良反应发生率为8.0%(4/50)与对照组28.0%(14/50)相比显著较低,且P<0.5。结论针对子宫内膜异位症患者采用米非司酮治疗的效果非常显著,既能减少患者复发率以及各种不良反应,还能提升其妊娠率,因此值得临床应用推广。

Objective To study and analyze the effect and mechanism of mifepristone in the treatment of endometriosis. Methods 100 cases of endometriosis in our hospital were selected as the subjects. The control group and the observation group were divided into the control group and the observation group, with 50 cases in each group. The control group was treated with dendazol at 1D, while the observation group was treated with mifepristone at 1D. The changes of serum CA125（serum cancer antigen 125）, clinical effi cacy, pregnancy rate, recurrence rate and adverse reaction were observed and compared between the two groups before and after treatment. Results There was no significant difference in serum CA125 level between the two groups before treatment, and P〈0.05. After treatment, the serum CA125 level in the observation group decreased more signifi cantly than that in the control group, and P〈0.05. The effective rate of the observation group was 94%（47/50）, and the difference was significant compared with the control group 80%（40/50）, and P〈0.05. The pregnancy rate in the observation group was 54%（27/50）, the recurrence rate was 15%（15/50）, and the difference was significant compared with the control group 40%（20/50） and 14%（7/50）, and P〈0.05. The incidence of adverse reaction in the observation group was 8%（4/50）, which was signifi cantly lower than that of the control group 28%（14/50）, and P〈0.5. Conclusion The effect of mifepristone in the treatment of endometriosis is very signifi cant. It can not only reduce the recurrence rate and various adverse reactions, but also improve the pregnancy rate, so it is worthy of clinical application and promotion.
