皮肤鳞状细胞癌临床特征及术后复发的相关因素分析

Correlative analysis between the clinical features and postoperative recurrence of cutaneous squamous cell carcinoma

目的分析影响皮肤鳞状细胞癌术后复发的相关因素。方法收集118例皮肤鳞状细胞癌患者资料,其中术后复发患者38例,无复发患者80例,分别对性别、年龄、发病病程、部位、肿瘤大小、肿瘤术前有无破溃、术后辅助放化疗、肿瘤病理分级、术后愈合情况、首次手术切缘情况、确诊时有无淋巴结转移等术后肿瘤复发相关因素进行统计学分析。结果肿瘤病理分级、术后愈合情况、确诊时有无淋巴结转移与肿瘤复发相关,经logistic回归显示:OR>1.6,P<0.05。此外,性别、年龄、发病病程、部位、肿瘤大小、肿瘤术前破溃、首次手术切缘情况、术后辅助放化疗等因素,对肿瘤复发影响尚不明确(P>0.05)。结论皮肤鳞状细胞癌术后复发与肿瘤病理分级、术后愈合情况、确诊时有无淋巴结转移有关。

Objective To analyze the related factors of postoperative recurrence of cutaneous squamous cell carcinoma. Methods 118 cases of cutaneous squamous cell carcinoma（SCC） were analyzed retrospectively.including 38 cases with recurrent,80 cases without recurrence.Sex,age,course of disease,location,tumor size and tumor breaking before operation were analyzed respectively.The factors related to postoperative tumor recurrence such as adjuvant radio-chemotherapy,pathological grade of tumor,postoperative healing,margin in first operation,lymph node metastasis at the time of diagnosis were analyzed. Results Pathological grade of tumor,postoperative healing,lymph node metastasis at the time of diagnosis were associated with tumor recurrence by Logistic regression （ OR 〉1.6; P〈 0.05）.In addition,sex,age,disease course,location,tumor size,tumor rupture before operation,the margin of the first operation and the adjuvant radiotherapy and chemotherapy after operation were analyzed by univariate and multivariate analysis.The results showed there were no significant difference in tumor recurrence（ P 〉0.05）. Conclusion The recurrence of cutaneous squamous cell carcinoma is associated with the pathological grade of the tumor,the healing after operation and the lymph node metastasis at the time of diagnosis.
