孟鲁司特钠咀嚼片治疗小儿肺炎支原体感染后慢性咳嗽的疗效及对尿白三烯的影响

Effect of Montelukast Sodium Chewable Tablets on Chronic Cough after Mycoplasma Pneumoniae Infection in Children and Its Effect on Urinary Leukotriene

目的:观察探讨孟鲁司特钠咀嚼片治疗小儿肺炎支原体感染后慢性咳嗽的临床效果及对尿白三烯的影响。方法:选取2015年8月-2017年8月本院收治的110例小儿肺炎支原体感染后持续性咳嗽的患儿,按照随机数字表法将其分为观察组和对照组,各55例。对照组采用阿奇霉素干混悬剂治疗,观察组在对照组基础上加用孟鲁司特钠咀嚼片治疗,观察比较两组患儿治疗4周后的临床效果、免疫指标及尿白三烯的变化。结果:观察组患儿治疗4周后总有效率为92.7%,高于对照组的70.9%,差异有统计学意义(X^2=8.497,P<0.05)。观察组患儿治疗后咳嗽和咳喘症状消退时间均明显短于对照组,差异均有统计学意义(t=6.293、8.144,P<0.05)。与治疗前相比,治疗4周后两组的Ig E、ECP、EOS计数与咳嗽评分、尿白三烯水平均明显降低(P<0.05),且观察组下降程度较对照组更为显著(P<0.05)。两组不良反应发生率比较,差异无统计学意义(X^2=0.34,P>0.05)。结论:孟鲁司特钠咀嚼片治疗小儿肺炎支原体感染后慢性咳嗽的临床效果显著,可以明显降低气道高反应及炎症反应,调节免疫系统,缩短病程,不良反应少,值得临床合理推广应用。

Objective：To observe and analyse the clinical effect of Montelukast Sodium Chewable Tablets on chronic cough in children with mycoplasma pneumoniae infection and its effect on urinary leukotriene.Method：110 children with persistent cough with mycoplasma pneumonia from August 2015 to August 2017 in our hospital were selected,according to the random number table method,they were divided into observation group and control group,55 cases in each group.The control group was treated with Azithromycin for Suspension,and the observation group was treated with Montelukast Sodium Chewable Tablets on the basis of the control group.The clinical effect,immune index and urinary leukotriene were observed and compared between the two groups after 4 weeks treatment.Result：After 4 weeks treatment,the total effective rate of the observation group was 92.7%,which was higher than 70.9% of the control group,the difference was statistically significant（ χ^2=8.497,P〈0.05）.The time of cough and cough relief after treatment in the observation group were significantly shorter than those in the control group,the differences were statistically significant（t=6.293、8.144,P〈0.05）.After treatment for 4 weeks,the IgE,ECP,EOS count and cough score,urinary leukotriene level in the two groups were significantly lower than those before treatment（P〈0.05）,the degree of decrease in the observation group was more significant than those in the control group（P〈0.05）.There was no significant difference in the incidence of adverse reactions between the two groups（ χ^2=0.34,P〉0.05）.Conclusion：Montelukast Sodium Chewable Tablets is effective in treating chronic cough after mycoplasma pneumoniae infection,it can significantly reduce airway hyperresponsiveness and inflammatory response, regulate immune system,shorten the course of disease and have fewer adverse reactions,which is worthy of clinical promotion and application.
