DR在评价儿童肘内翻指标及肘内翻分度的应用

Application of DR in Evaluation of Children Cubitus Varus Index and Elbow Varus Degree

目的:探讨DR在评价儿童肘内翻指标及肘内翻分度的应用。方法:搜集6~14岁单侧发生肘内翻对侧正常病例60例,摄双肘关节标椎正侧位片对比。在测量携带角的基础上,分别增加测量肱骨角、尺骨角、肱骨外髁骺线角,总结出各角正常范围值,并根据各角大小及肘关节功能进行肘内翻的分度。结果:60例单肘内翻病例中,43例轻度内翻(71%),9例中度内翻(15%),8例重度内翻(13%)。正常各角因年龄及性别不同跨度较大,正常值分别是:携带角男2°~26°,女2°~22°;肱骨角男77°~95°,女72°~91°;尺骨角男77°~99°,女72°~93°;肱骨外髁骺线角与携带角基本一致,但是大龄儿童该角的增大要比携带角明显。骨折复位后测量该角有助于预测携带角的大小,肱骨角、尺骨角与携带角没有直接关联性,但该角增大或减小可以提示内外翻的发生。结论:DR标准肘关节正侧位片(摄健侧对比分析)可以有效的对肘内翻进行评价及分度,并通过测量各项指标更客观的评判肘内翻程度及指导临床治疗。

Objective： To explore the application of evaluation index of children cubitus varus index and elbow varus degree. Methods：To collect 6 ~ 14 years old after unilateral elbow varus contralateral normal cases 60 cases was taken double elbow the vertebral side comparison. In the basis of the measurement of angle,respectively to increase the measuring angle of humerus,ulna Angle,humerus condyle epiphyseal line angle,summarizing the angle of the normal range value,and depending on the function of elbow joint and the size of the angle of elbow varus index. Results： In 60 cases of single elbow varus,43 cases of mild pronation（ 71%）,9 cases were moderate varus（ 15%）,8 cases of severe pronation（ 13%）. Normal angle of each different span larger because of age and sex. The normal value of the male carrying angle was 2° ~ 26°,the female was 2° ~ 22°. The male Humerus angle was 77° ~ 95°,the female was 72° ~ 72°. The male Angle of ulna was 77° ~ 99°,the femalewas 72° ~ 72°. Humerus lateral condyle epiphyseal line angle and the carrying angle,but in the increase of the angle than older children carrying angle; Fracture after measuring the angle can help predict the size of carrying angle,angle of the humerus,ulna angle had no direct relevance with the carrying angle,but the angle increase/decrease could prompt inside and outside. Conclusion： The anterioposterior and lateral film of DR standard elbow（ pictured the contralateral analysis） could effectively evaluate and degree the cubitus varus,and objective evaluation of elbow varus and guide clinical treatment by measuring the each index.
