糖尿病微血管病变患者血液指标改变的意义

The clinical significance of changes of some blood parameters in diabetic patients with microangiopathy

目的:探讨糖尿病微血管病变患者的血小板参数、纤维蛋白原和D-二聚体指标改变的意义。方法:对118例糖尿病患者(糖尿病组,其中单纯糖尿病67例为A组,微血管病变51例为B组)和110例健康体检者(对照组,C组)的血小板参数、纤维蛋白原和D-二聚体进行测定并比较。结果:糖尿病组患者平均血小板体积、血小板分布宽度、血小板压积与纤维蛋白原和D-二聚体指标高于对照组,血小板计数指标低于对照组,两组比较差异均具有统计学意义(P<0.05)。单纯糖尿病组血小板参数、纤维蛋白原和D-二聚体指标均低于微血管病变组,差异有统计学意义(P<0.05)。结论:糖尿病患者的平均血小板体积、血小板分布宽度、血小板压积、纤维蛋白原和D-二聚体的表达普遍高于正常人,血小板计数低于正常人。患有微血管病变的糖尿病患者的血小板参数、纤维蛋白原与D-二聚体的表达普遍高于单纯糖尿病患者,故血小板参数、纤维蛋白原和D-二聚体对糖尿病的联合测定有利于糖尿病微血管病变的早期诊断。

Objective To investigate the clinical significance of changes of platelet parameters,fibrinogen and D-Dimer in diabetic patients with microangiopathy. Method 118 diabetic patients（ diabetic group,67 simple diabetic patients as Group A,51 patients with diabetic microangiopathy as Group B） and 110 healthy people＇s（ control group,Group C） platelet parameters,fibrinogen and D-Dimer were detected and compared. Results The mean platelet volume,platelet distribution width,thrombocytocrit,fibrinogen and D-Dimer of diabetic group were higher than control group,the platelet count was lower than control group,all of the difference was statistically significant（ P〈0.05）. And the platelet parameters,fibrinogen and D-Dimer of simple diabetic patients were lower than patients with diabetic microangiopathy,all of the difference was statistically significant（ P〈0.05）. Conclusion The expression of mean platelet volume,platelet distribution width,fibrinogen and D-Dimer of diabetic patients is generally higher than normal,the platelet count is lower than normal; and the expression of platelet parameters,fibrinogen and D-Dimer of patients with diabetic microangiopathy is generally higher than simple diabetic patients,so the simultaneous determination of platelet parameters,fibrinogen and D-Dimer is good for the determination of the early diagnosis of diabetic microangiopathy.
