耻骨联合分离为主要表现的TileB1型骨盆损伤两种内固定的力学稳定性研究

Stability study of using plate or hollow screw to fix Tile B1 pelvic injury with pubic symphysis diastasis as main damage characteristic

目的:研究耻骨联合分离两种内固定所提供力学稳定性的差异。方法:8具防腐骨盆标本,载负荷测试两种体位下耻骨联合在不同方向的相对位移;制备实验骨盆损伤模型,复位后重建钢板或空心螺钉固定,分别载负荷测试两种体位下耻骨联合在不同方向的相对位移。分析两种固定方式的生物力学稳定性,并比较分析。结果:力学研究依次测试完毕,三组标本及构建模型无骨折、断裂及松动等异常。骨盆标本生物力学研究:双足、单足静止站立位,耻骨联合水平、前后及上下方向三组间比较有统计学差异(P<0.05)。结论:骨盆标本生物力学研究:在对抗耻骨联合水平、前后方向的张应力空心螺钉比钢板提供更强力学稳定性,在对抗上下方向剪切应力重建钢板较空心螺钉提供更坚强的稳定性。

Objective：The difference of mechanics stability between two kinds of internal fixation which fixed the pubic symphysis diastasis werespecimen of antisepticized pelvises have been obtained.The displacement of pubic symphysis on three orientations were measured when the force was loaded on lumbar vertebra under both feet or single foot static standing position.The Tile B1 pelvic injury models according to experimental request were made,then the pubic symphysis was fixed with reconstruction plate or hollow screw after reduction,finally the displacement of pubic symphysis were measured with the same method as above.All data were analyzed andfracture or rupture or looseness weren＇t found in all specimen and restructed models after mechanics measured.The displacement of pubic symphysis on three orientations among three groups haven’t statistical diffence（P〈0.05）on the biomechanical study of pelvicbiomechanical measurement of Tile B1 injury pelvic specimen showed that the pubic symphysis stability of hollow screw fixation is better than plate fixation on horizontal and sagittal orientation.The pubic symphysis stability of plate fixation is better than hollow screw fixation on vertical orientation,especially,under single foot static standing position.
