某中医院消化科323株病原菌的分布及耐药分析

Distribution and Drug Resistance Analysis of 323 Strains of Pathogenic Bacteria in Gastroenterology Department of a TCM Hospital

摘要 调查中医院消化科临床常见病原菌的分布及耐药性。回顾分析消化科分离病原菌,病原菌鉴定和药敏实验采用VITEK2分析仪。结果表明:消化科共分离323株病原菌,构成比69.35%为革兰氏阴性菌、26.32%为革兰氏阳性菌、4.33%为真菌。标本以痰液及血液为主,位于前5位的病原菌依次为大肠埃希菌、鲍曼不动杆菌、肺炎克雷伯菌、铜绿假单胞菌、表皮葡萄球菌。耐甲氧西林表皮葡萄球菌分离率为76.0%,未检测出耐万古霉素和耐利奈唑胺的葡萄球菌及肠球菌株。肠杆菌科细菌除青霉素、一代头孢耐药率较高外,对其他常用药物耐药率较低。铜绿假单胞菌对氨基糖苷类、碳青酶烯类、喹诺酮类抗菌药及哌拉西林/他唑巴坦的耐药率低于15%。鲍曼不动杆菌表现多重耐药,耐药率高于80%,只有替甲环素、多粘菌素不耐药。消化科细菌耐药情况尚不严重,但需高度重视鲍曼不动杆菌、嗜麦芽窄食单胞菌、耐甲氧西林葡萄球菌的情况,减少耐药菌株的传播。 To investigate the distribution and drug resistance of clinical common pathogenic bacteria in the Gastroenterology Department of TCM hospital.Method:Retrospective analysis of pathogenic bacteria separation of Gastroenterology Department,pathogenic bacteria identification and VITEK2 analyzer used for the drug sensitive test.Result:The Gastroenterology Department separated a total of 323 strains of pathogenic bacteria.The constituent ratio was 69.35% gram-negative bacteria,26.32% gram-positive bacteria and 4.33% fungi.The specimens were mainly sputum and blood,and the first 5 pathogenic bacteria were escherichia coli,acinetobacter baumannii,klebsiella pneumoniae,Pseudomonas aeruginosa,pseudomonas aeruginosa and staphylococcus epidermidis.The separation rate of methicillin-resistant staphycoccus epidermidis was 76.0%,and the staphylococcus and enterococcus strains resistant to vancomycin and linezolid were not detected.Except for penicillin and generation-I cephalosporin,the enterobacteriaceae has low drug resistance rate to other common drugs.The drug resistance rate of pseudomonas aeruginosa to aminoglycosides,carbapenem,quinolones and piperacillin/tazobactam was less than 15%.Except for methacycline and polymyxin,the acinetobacter baumannii showed multi-drug resistance,and the drug resistance rate was higher than 80%.Conclusion:The drug resistance of bacteria in Gastroenterology Department was not serious,but it is necessary to attach great importance to the situation of acinetobacter baumannii,stenotrophomonas maltophilia and methicillin-resistant Staphylococcus,to reduce the spread of drug resistant strains.
