Assessment of effect of combined hemoperfusion and hemodialysis in treatment of acute paraquat poisoning:a meta-analysis

摘要 [目的]分析血液透析联合血液灌流医治急性百草枯中毒的疗效。[方法]通过检索CBMdisc、CNKI、CHKD、万方、维普、Pub Med、Embase等国内外数据库近16年公开发表的血液透析联合血液灌流治疗百草枯中毒的相关论文,使用Rev Man5.3软件对结果进行meta分析。[结果]共检索出文献317篇,9篇文献纳入。共计543例患者,其中血液透析联合血液灌流治疗组(治疗组)308例,死亡90例,常规治疗组(对照组)235例,死亡156例。各研究间同质性较好(χ2=5.96,DF=8,P>0.05,I2=0%),疗效的合并OR值=0.19(95%CI为0.13~0.28,Z=8.51,P<0.01)。[结论]血液透析联合血液灌流医治急性百草枯中毒相比常规治疗,可显著降低死亡率。但确切疗效还需考虑其他多种因素,需要更多数据,在临床上进一步探究。 [Objective]To observe the therapeutic effect of combined hemoperfusion and hemodialysis in the treatment of acute paraquat poisoning.[Methods]The papers related to therapy of acute paraquat poisoning with combined hemoperfusion and hemodialysis in the databanks,including CBMdisc,CNKI,CHKD,CSPD,CQVIP,Pub Med and Embase databases in recently 16 years were searched.The available data was analyzed by the meta-analysis software of Rev Man 5.3.[Results]Finally 9 research papers were used among 317 searched papers.Totally 543 cases were reported,among which 308 cases were treated with combined hemodialysis and hemoperfusion and 235 cases were treated in routine way,and 90 death cases were reported in the treated group while 156 death cases reported in the routine treatment group.There was a good homogeneity of each=5.96,DF=8,P〉0.05,I2=0%).The combined OR of efficacy was 0.19(95%CI:0.13-0.28,Z=8.51,P〈0.01).[Conclusion]The therapy of combined hemoperfusion and hemodialysis seems effective in the treatment of acute paraquat poisoning,but more data is needed to confirm such effects when a number of other confounding factors are controlled.
