黄芪建中汤加减对慢性肾炎患者Scr、Bun、24hUP的影响

Effect of Modified Huangqi Jianzhong Decoction on Level of Scr,Bun and 24hUP in Patients with Chronic Nephritis

目的:探讨黄芪建中汤加减治疗慢性肾炎脾肾阳虚证的临床疗效及对患者24小时尿蛋白定量(24 hours urinary protein,24h UP)、尿素氮(urea nitrogen,Bun)及血肌酐(serum creatinine,Scr)的影响。方法:将慢性肾炎患者102例随机分为对照组51例和治疗组51例。对照组口服盐酸贝那普利片,每次10 mg,每天1次;双嘧达莫片,每次50 mg,每天3次;血脂异常者口服阿托伐他汀钙片,每次10 mg,每天1次,晚餐时服用。治疗组在对照组基础上采取黄芪建中汤加减治疗,每天1剂,常规水煎分2次温服。两组患者均连续治疗16周。比较两组患者脾肾阳虚证症状评分、临床疗效。检测两组患者24h UP、Bun及Scr水平。运用中文版健康调查简表(36-item short form health survey,SF-36)评价两组患者生活质量。结果:治疗后,治疗组脾肾阳虚证症状评分显著低于对照组(P<0.01)。治疗组有效率为92.16%,明显高于对照组的74.51%(P<0.05)。治疗后,治疗组Scr、Bun、24h UP明显低于对照组(P<0.01)。治疗组治疗后SF-36各指标积分显著高于对照组(P<0.01)。结论:黄芪建中汤加减治疗慢性肾炎脾肾阳虚证,可减轻患者的临床症状,提高临床疗效,改善患者的Scr、Bun、24h UP水平和生活质量。

Objective：To investigate the clinical efficacy of modified Huangqi Jianzhong Decoction in treatment of chronic nephritis with Spleen-kidney Yang Deficiency Syndrome and the effect on 24 h urinary protein（24 hU P）,urea nitrogen（ Bun） and serum creatinine（ Scr） levels. Methods： 102 patients with chronic nephritis were randomly divided into control group and treatment group,with 51 cases in each group. The control group was treated with Benazepril hydrochloride tablets,10 mg each time,once a day;dipyridamole tablets,50 mg each time,3 times a day;dyslipidemia oral atorvastatin calcium tablets,10 mg each time,1 day Times,taken at dinner. The treatment group was treated with modified Huangqi Jianzhong Decoction in addition to that of the control group. One dose per day with conventional boiled for 2 times. Both groups were treated continuously for 16 weeks. The symptom scores and clinical efficacy of the two groups were compared. The levels of 24 hU P,Bun,and Scr were detected in both groups. A36-item short form health survey（ SF-36） was used to evaluate the quality of life of the two groups of patients. Results：After treatment,the symptom score in the treatment group were significantly lower than that in the control group（ P〈0. 01）. The effective rate in the treatment group was 92. 16%,which was significantly higher than that（74. 51%） in the control group（ P〈0. 05）. After treatment,Scr,Bun,and 24 hU P in the treatment group were significantly lower than those in the control group（ P〈0. 01）. SF-36 scores in the treatment group were significantly higher than those in the control group after treatment（ P〈0. 01）. Conclusion：Huangqi Jianzhong Decoction in treatment of chronic nephritis with Spleen-kidney Yang Deficiency Syndrome can reduce the clinical symptoms,improve the clinical efficacy,and improve the level of Scr,Bun,24 hU P and patients＇ quality of life.
