益气温中针灸疗法联合益气聪明汤治疗气血亏虚型颈性眩晕临床研究

Clinical Study on Effect of Yiqi Wenzhong Acupuncture and Moxibustion Treatment Combined with Yiqicongming Decoction in Treatment of Qi and Blood Deficiency Type Cervical Vertigo

目的:观察益气温中针灸疗法联合益气聪明汤治疗气血亏虚型颈性眩晕的临床疗效。方法:将70例气血亏虚型颈性眩晕患者随机分成两组,每组35例。所有患者均给予常规治疗,对照组给予益气温中针灸疗法,治疗组在此基础上加用益气聪明汤治疗,测定两组患者治疗前后血清纤维蛋白原(serum fibrinogen,Fib)及血清总胆固醇(total cholesterol,TC)水平,检测椎-基底动脉血流速度及血管搏动指数(pulsatility index,PI),眩晕评分及椎动脉型颈椎病功能评定量表(functional scale for CSA,FS-CSA)评分评估眩晕程度,比较临床疗效。结果:与治疗前比较,两组患者血清Fib及TC水平均显著降低(P<0.01),左椎动脉、右椎动脉以及基底动脉最大血流速度均明显升高(P<0.05),PI明显降低(P<0.05),眩晕评分显著升高(P<0.01),FS-CSA评分显著降低(P<0.01)。与对照组比较,治疗组患者血清Fib及TC水平显著降低(P<0.01),左椎动脉、右椎动脉以及基底动脉最大血流速度显著升高(P<0.05),眩晕评分显著升高(P<0.01),FSCSA评分显著降低(P<0.01),治疗有效率显著升高(P<0.05)。结论:益气温中针灸疗法联合益气聪明汤治疗气血亏虚型颈性眩晕疗效确切,可能与降低血清Fib及TC水平有关。

Objective：The effect of Yiqi Wenzhong acupuncture and moxibustion treatment combined with Yiqicongming Decoction in the treatment of Qi and blood deficiency type cervical vertigo. Methods：70 cases of blood deficiency type cervical vertigo were randomly divided into two groups,35 cases of each. All patients were given general treatment,the control group was given acupuncture therapy in Yiqi Wenzhong,and the treatment group based on the control group was given Yiqicongming decoction. Serum fibrinogen（ Fib） and total cholesterol（ TC） level were determined before after the treatment,vertebral-basilar artery blood flow velocity and pulsatility index of pulsatility index（ PI）,vertigo score and functional scale for CSA（ FS-CSA） score evaluation degree of vertigo and clinical efficacy were compared after the treatment. Results：Compared with before treatment,serum Fib and TC levels of the two groups significantly decreased（ P〈0. 01）,the left vertebral artery,right vertebral artery and basilar artery blood flow velocity significantly increased（ P〈0. 05）,and the PI decreased（ P〈0. 05）,vertigo score significantly increased（ P〈0. 01）,and FS-CSA score decreased significantly（ P〈0. 01）;compared with the control group,serum levels of FIB and TC level in the treatment group were lower（ P〈0. 01）,the left vertebral artery,right vertebral artery and basilar artery blood flow velocity were higher（ P〈0. 05）,vertigo score was higher（ P〈0. 01）,and FS-CSA score was lower（ P〈0. 01）,and the effective rate in the treatment was higher（ P〈0. 05）. Conclusion：Yiqi Wenzhong acupuncture and moxibustion treatment combined with Yiqicongming Decoction in treatment of Qi and blood deficiency type cervical vertigo was effective,and the mechanism was related to reduced the levels of serum TC and Fib.
