摘要
目的评估目前关于黄连素治疗慢性心衰的有效性及其对LVEF、6分钟步行实验的影响。方法我们全面搜索CNKI、万方、Pub Med、Cochrane图书馆数据库,检索时间截止2017年1月。对纳入的RCT研究用Cochrane手册进行偏倚风险评估,用Rev Man 5.3软件进行Meta分析。结果 7项RCT研究包括562例慢性心力衰竭患者,Meta分析结果提示加用黄连素组在总有效率(OR:3.64;95%CI,1.79~7.44;P<0.01)、提高LVEF(MD:4.78;95%CI,3.16~6.40;P<0.01)、改善6分钟步行试验(MD:34.78;95%CI,22.37~47.19;P<0.01)优于对照组。结论在基础治疗上加用黄连素可提高慢性心力衰竭的疗效,提高LVEF及改善6分钟步行试验。但由于所纳入的文献质量普遍不高,结论有待更高质量、大样本、多中心随机对照试验以进一步更新及确认。
To evaluate the efficacy of berberine in the treatment of chronic heart failure and its effect on LVEF and 6-minute walk. Methods We searched the database of CNKI, Wanfang, PubMed and Cochrane library comprehensively, and the retrieval time was up to January 2017. The included RCT study was assessed with the Cochrane manual for bias risk, and Meta-analysis was performed with Rev Man 5.3 software. Results 7 RCT studies included 562 patients with chronic heart failure. "Meta analysis results suggest combined with berberine group in the total effective rate (OR:3.64, 95% CI, 1.79 to 7.44, P 〈 0.01) was increased in the berberine group, the LVEF (MD:4.78, 95% CI, 3.16-6.40, P〈 0.01) were better than those in the control group (P〈 0.01), the 6 minutes walking test (MD:34.78, 95% CI, 22.37-47.19, P 〈 0.01)" was better than the control group. Conclusion Adding berberberine to basic treatment can improve the curative effect of chronic heart failure, improve LVEF and improve 6-minute walking test. However, due to the generally low quality of the included literature, the conclusions need to be further updated and confirmed by higher quality, large sample and multi-center randomized controlled trials.
