超声弹性成像与超声造影在乳腺癌新辅助化疗效果评估中的应用

Application of ultrasound elastography and contrast-enhanced ultrasonography in the evaluation of neoadjuvant chemotherapy for breast cancer

目的探讨超声弹性成像与超声造影检查技术在乳腺癌新辅助化疗效果评估中的应用价值。方法选择2015年1月~2017年1月于台州市中西医结合医院接受新辅助化疗的55例乳腺癌患者作为研究对象。所有患者均接受新辅助化疗,并在化疗周期前后进行超声弹性成像、超声造影检查,对比成像诊断结果与病理结果的差异,明确两种检查方案在乳腺癌新辅助化疗效果评价中的价值。结果化疗4个周期后超声弹性成像、超声造影评价结果与病理证实比较差异无统计学意义(P>0.05);超声造影检查的灵敏度、准确度与超声弹性成像比较差异无统计学意义(P>0.05)。结论超声弹性成像与超声造影检查在评估乳腺癌新辅助化疗效果中均具有较高的灵敏度与准确性,但二者在评价方向与内容方面不同,临床可结合弹性成像与超声造影检查结果,进一步提高乳腺癌新辅助化疗效果评估的准确性。

Objective To discuss the comparison of ultrasound elastography and contrast-enhanced ultrasonography in the evaluation of neoadjuvant chemotherapy for breast cancer.Methods A total of 55 patients with breast cancer in Chinese and Western Medicine Hospital of Taizhou City from January 2015 to January 2017 were selected,all patients underwent neoadjuvant chemotherapy,ultrasound elastic imaging and ultrasound angiography were used in patients before and after the chemotherapy cycle,the differences between the imaging results and pathological results were compared,and the application of two kinds of examination regimen in the evaluation of neoadjuvant chemotherapy for breast cancer were determined.Results There were not significant differences in ultrasound elastography,ultrasonography,and pathological results after 4 cycles of chemotherapy（P〈0.05）.Compared with ultrasound elastography,there was no significant difference in the sensitivity and accuracy of CEUS（P 0.05）.Conclusion There is a high sensitivity and accuracy in the evaluation of neoadjuvant chemotherapy in breast cancer with ultrasound elastography and contrast-enhanced ultrasound,and it is different in the evaluation of the direction and content.Combined with elastic imaging and ultrasound angiography in the clinical can improve the accuracy assessment of neoadjuvant chemotherapy in breast cancer.
