罗伊适应护理模式在先心病患儿家属干预中的应用效果

Application effect of Roy adaptive nursing intervention in family members of children with congenital heart disease

目的探讨罗伊适应护理模式在先心病患儿家属干预中的应用效果。方法纳入2017年6~9月于广州市妇女儿童医疗中心心外科治疗的150例先天性心脏病手术患儿,采用随机数字表将患儿分为研究组与对照组,每组各75例。对照组患儿家属采取常规护理措施干预,包括病情观察、基础护理、对患儿家属进行健康指导等。研究组在此基础上于围术期实施罗伊适应护理模式,两组干预时间均为1个月。采用住院护理满意度调查问卷评价两组患儿家属对护理服务的满意程度情况,采用抑郁自评量表(SDS)、焦虑自评量表(SAS)了解两组患儿家属的心理健康状态。结果对照组患儿家属对护理工作的满意率为85.45%,研究组为98.18%,差异有统计学意义(P<0.05)。两组干预后SDS、SAS评分较干预前均显著下降,且研究组SDS、SAS评分均低于对照组,差异有统计学意义(P<0.05)。结论对心脏病手术患儿实施罗伊适应护理模式不仅可以为手术创造良好条件,还可以提高患儿及其家属对护理服务的满意程度,使他们在就医过程中获取到安全感与满足感,降低了患儿家属对手术焦虑与担忧心理,提高了医疗质量。

Objective To investigate the effect of Roy adaptive nursing intervention on family intervention in children with congenital heart disease.Methods A total of 150 children with congenital heart disease who were treated in Department of Cardiac Surgery,Guangzhou Women and Children Medical Center from June to September 2017 were selected and randomly divided into study group and control group according to random number table,with 75 cases in each group.The relatives of children in the control group were given routine nursing intervention,including observation of the disease,basic nursing,and health guidance for the family members of the children.On the basis of the control group,the study group was given Roy adaptive nursing intervention during perioperative period.Both the two groups were intervened for 1 month.The satisfaction degree of nursing service was evaluated by the questionnaire of hospital nursing satisfaction.SDS and SAS were used to investigate the mental health status of the two groups.Results 85.45%of the children′ s family members in the control group was satisfied with the nursing work,which was 98.18% in the study group,the difference was statistically significant（P〈0.05）.The scores of SDS and SAS after intervention were significantly decreased compared with before intervention in the two groups（P 0.05）.The scores of SDS and SAS after intervention in the study group were lower than those in the control group,and the differences were statistically significant（P 0.05）.Conclusion Implementing Roy adaptive nursing model in children with cardiac surgery can not only create favorable conditions for surgery,but also improve the satisfaction of nursing services for children and their families,and make them feel safe during the medical treatment,reduce the anxiety for surgery of the relatives of children,thus improve the quality of care.
