Changes and significance of adhesion molecule and cell proliferation index before and after chemotherapy in non-small cell lung cancer

摘要 目的探讨非小细胞肺癌(NSCLC)化疗前后黏附分子及血管新生指标水平变化及意义。方法 90例NSCLC患者设为观察组,依据TNM分期可分为Ⅰ期~Ⅱ期(n=19)、Ⅲ期(n=48)、Ⅳ期(n=23),另选取同期来院健康体检者90例作为对照组。比较对照组与观察组各分期间,观察组化疗前后黏附因子及细胞新生指标水平。结果观察组ICAM-1、VCAM-1、IGF-1及EGFR水平均显著高于对照组(P<0.05),且不同TNM分期间指标水平差异有统计学意义(P<0.05),ICAM-1、VCAM-1、IGF-1及EGFR水平与肿瘤分期呈显著正相关性(P<0.05);化疗后观察组ICAM-1、VCAM-1、IGF-1及EGFR水平较化疗前显著降低,且仍显著高于对照组,差异有统计学意义(P<0.05)。结论 NSCLC患者血清黏附分子及细胞新生指标异常升高,其指标水平检测对NSCLC的诊断,分期及化疗指导等具有重要价值。 Objective To investigate the changes and significance of adhesion molecules and angiogenesis indexes in patients with non-small cell lung cancer (NSCLC) before and after chemotherapy. Methods A total of 90 patients with NSCLC were selected as the observation group, according to TNM stage they were divided into stage Ⅰ-Ⅱ( n=19 ), stage Ⅲ( n=48 ), stage Ⅳ ( n=23 ) , and another 90 cases of healthy people were selected as control group in the same period. The levels of adhesion molecules and angiogenesis indexes between the control group and observation group, and the different staging of observation were compared. Results The ICAM-1, VCAM-1, IGF-1 and EGFR levels of observation group were significantly higher than those in the control group (P〈0.05) , and the difference of levels between different stagings were statistically significant(P〈0.05). The levels of ICAM-1, VCAM-1, IGF-1 and EGFR were positively correlated with staging (P〈0.05). After chemotherapy, the levels of ICAM-l, VCAM-1, IGF-1 and EGFR in the observation group significantly decreased compared with those before chemotherapy, but still significantly higher than those in the control group, and the differences were statistically significant (P〈0.05). Conclusion Serum adhesion molecules and cell regeneration indexes are abnormally elevated in patients with NSCLC, and the detection of their indexes is of great value in the diagnosis, staging and prognosis evaluation.
