临床标本与食品样品副溶血性弧菌分离株耐抗生素Meta分析

摘要 目的:评价我国临床标本中分离到的副溶血性弧菌的耐药性与食品来源的菌株的差异。方法:检索2007年1月至2017年7月中国期刊网、维普数据库、万方数据库有关临床标本和食品样品分离到的副溶血性弧菌耐药性检测的文献,根据纳入和排除标准筛选符合要求的研究提取数据,以不同来源菌株对单种的耐药率为效应量,应用Review manager 4.2软件进行meta分析。结果:共纳入9个研究,涉及294株腹泻标本来源和226株食品来源分离株进行耐药率的meta分析;根据纳入文献,腹泻标本和食品样品分离株主要对6种抗生素具耐药性,其中对5种有耐药性的抗生素(氨苄西林、羧苄西林、头孢噻吩、头孢呋辛钠、链霉素)耐药率差异无统计学意义,对头孢唑啉的耐药率差异有统计学意义。结论:在我国,副溶血性弧菌对于主要抗生素的耐药性并未经肠道获得明显增强,但抗生素滥用对出现副溶血性弧菌耐药增强的隐患仍然不容忽视。 Objective: To evaluate the antibiotic resistance difference of Vibrio parahaemolyticus(VP) separated from the clinical specimens and food. Methods: The concerned data was extracted from the literatures about the antibiotic resistance of VP. The literatures were from in full-text database of CNKI, VIP and Wanfang Database from January 2007 to July 2017 and screened out according to the inclusion and exclusion criteria. The data was mate-analyzed by Review Manager 4.2 software through setting the single antibiotic resistance rate of different resource bacteria as the effect size. Results A total of 9 researchs were included for Meta-analysis, involving 294 strains from patients with diarrhea and 226 strains from food samples. The isolates mainly show antibiotic resistance in 6 antibiotics, for 5 of them(ampicillin, carbenicillin, ceftriaxone, cefuroxime sodium, streptomycin) it was no significant statistical difference in antibiotic resistance rate of the isolates, but for cefazolin it was statistical difference. Conclusions: In our country, the resistance of VP to the main antibiotics is not significantly enhanced by intestinal tract. However, the potential perniciousness of the antibiotic resistance of VP caused by antibiotic abuse is still not to be ignored.
