BiPAP呼吸机无创通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭效果分析 被引量:7

摘要 目的探讨慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者应用双水平式呼吸道正压(Bi-level positive airway pressure,Bi PAP)呼吸机无创通气治疗的临床疗效。方法随机选取78例自2016年10月~2017年10月间在我院接受诊治的慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者,将其分为研究组和对照组,对照组患者应用常规治疗,研究组患者同时应用无创呼吸机辅助通气治疗。比较两组治疗前后血气值[动脉血氧饱和度(Saturation of arterial oxygen,Sa O2)、动脉血二氧化碳分压(Partial pressure of arterial carbon dioxide,Pa CO2)、动脉血氧分压(Partial pressure of arterial oxygen,Pa O2)、酸碱度(hydrogen ion concentration,p H)]、呼吸频率及心率。结果治疗后两组患者Sa O2、Pa O2、p H值均明显高于治疗前,Pa CO2低于治疗前。治疗后研究组患者各指标均明显优于对照组,差异存在统计学意义(P<0.05)。治疗后两组患者呼吸频率和心率均较治疗前下降,且研究组患者下降幅度更为明显,差异存在统计学意义(P<0.05)。结论慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者应用Bi PAP呼吸机无创通气治疗有助于缓解临床症状,改善生活质量,疗效显著。 Objective To investigate the clinical efficacy of Bi-level positive airway pressure(Bi PAP) non-invasive ventilation in patients with chronic obstructive pulmonary disease and type Ⅱ respiratory failure. Methods A total of 78 patients with chronic obstructive pulmonary disease(COPD) and type Ⅱ respiratory failure who were diagnosed and treated in our hospital from October 2016 to October 2017 were randomly divided into study group and control group.Patients in control group were treated with routine treatment, and the study group was treated with noninvasive ventilator-assisted ventilation. The blood gas values including Saturation of Arterial Oxygen(Sa O2), Partial Pressure of Arterial Carbon Dioxide(Pa CO2), Partial Pressure of Arterial Oxygen(Pa O2), hydrogen ion concentration(p H), respiratory rate and heart rate between two groups before and after treatment were compared. Results The Sa O2, Pa O2 and p H values of two groups after treatment were significantly higher than those before treatment. And Pa CO2 of two groups after treatment was lower than that before treatment. After treatment, the indicators of the study group were significantly better than those of the control group, and the difference was statistically significant(P〈0.05). After treatment, the respiratory rate and heart rate of the two groups decreased compared with those before treatment, and the decrease rate of the study group was more obvious, with statistically significant difference(P〈0.05). Conclusion Bi PAP noninvasive ventilation in patients with chronic obstructive pulmonary disease and type Ⅱ respiratory failure can help relieve clinical symptoms and improve quality of life, which has significant effect.
