达芬奇机器人手术系统在66例妇科恶性肿瘤手术中的应用分析

Application of Da Vinci robotic surgical system in 66 cases of gynecologic malignancies

目的探讨达芬奇机器人手术系统在妇科恶性肿瘤手术中应用的安全性及有效性。方法选择2016年9月至2018年1月本院妇产科收治的166例恶性肿瘤患者为研究对象,采用抽签法将其分为机器人组(66例)和腹腔镜组(100例)。机器人组患者采用达芬奇机器人手术治疗,腹腔镜组患者采用腹腔镜手术治疗,观察两组患者的手术时间、术中出血量、清扫淋巴结数、术后48小时引流量、术后通气时间、术后住院天数。结果机器人组患者术中出血量、术后48小时腹腔引流量、术后通气时间、术后住院天数、并发症发生率均显著低于腹腔镜组(P<0.05),手术时间显著长于腹腔镜组(P<0.05),清扫淋巴结数明显多于腹腔镜组(P<0.05)。结论在妇科恶性肿瘤的治疗中,达芬奇机器人辅助腹腔镜手术能够提高手术质量,缩短患者恢复时间,降低并发症发生率,提高治疗效果。

Objective To explore the safety and effectiveness of Da Vinci robotic surgical system in gynecologic malignancies surgery. Method 166 patients with gynecologic malignant tumor from September 2016 to January 2018 in our hospital were selected as objects of study, they were divided into robot group（66 cases） and laparoscopic group（100 cases） by lottery method. The robot group was treated with Da Vinci robotic surgical system and the laparoscopic group was treated with laparoscopic surgery treatment. The operation time, intraoperative blood loss, the amount of drainage 48 hours after operation, dissection number of lymph node, aeration time, postoperative hospitalization days after surgery of the two groups were observed. Result The amount of bleeding during the operation, volume of abdominal drain 48 hours after operation, postoperative ventilation time, postoperative hospital days, the incidence of complications of robot group were significantly lower than those of laparoscope group（P〈0.05）; the operative time was significantly longer than laparoscopic group（P〈0.05）, the number of lymph nodes dissected in robot group was significantly more than laparoscopic group（P〈0.05）. Conclusion In the treatment of gynecologic malignant tumors, Da Vinci robotic surgical system assisted laparoscopic surgery can improve the quality of operation, reduce patients recovery time and the incidence of complications and improve treatment effects.
